腹腔镜与胆道镜联合取石术治疗肝内外胆管结石分析

The Analysis of Laparoscopic and Endoscopic Biliary Lithotripsy in the Treatment of Extrahepatic Bile Duct Stones

目的:探讨腹腔镜联合胆道镜取石术对于治疗肝内外胆管结石的临床价值。方法:选择笔者所在医院收治的确诊为肝内外胆管结石的160例患者作为研究对象并进行分组治疗,试验组所有患者应用腹腔镜联合胆道镜取石术进行治疗,对照组所有患者则采用开腹手术进行治疗。将治疗效果和有关治疗数据进行对比。结果:在平均手术所需时间方面,试验组为(160±15)min,对照组为(150±30)min;在平均术中出血量方面,试验组为(20±10)ml;显著低于对照组的(180±75)ml;在平均住院时间方面,试验组为(6±2)d,显著低于对照组的(10±1)d;在结石清除率方面,试验组为93.33%,高于对照组的91.43%;在术后并发症方面,试验组为0,对照组有4例胆漏。两组患者术后6个月均无复发。结论:腹腔镜联合胆道镜取石术安全有效,患者恢复快,值得广泛应用。

Objective：To investigate clinical value of the combined laparoscopic and endoscopic biliary lithotripsy in the treatment of extrahepatic bile duct stones.Method：160 cases of extrahepatic bile duct stones in our hospital were selected as research subjects and were divided into two groups,patients in the experimental group all were treated with combined laparoscopic and endoscopic biliary lithotripsy,all patients in the control group were treated with open cholecystectomy.The therapeutic effect of the treatment and the comparison data were compared.Result：The mean operative time of the experimental group was （160±15）min,it was more than （150±30）min of the control group;the average amount of bleeding in the experimental group was （20±10）ml,it was significantly lower than （180±75）ml of the control group,the average length of stay in the experimental group was （6±2）days,it was significantly lower than （10±1）days of the control group,stone clearance rate of the experimental group was 93.33%,it was significantly higher than the 91.43%of the control group,postoperative complications in the experimental group was 0,there were four cases of bile leakage in the control group.Two groups of patients had no recurrence after 6 months. Conclusion：The laparoscopic and endoscopic biliary lithotripsy is safe and effective,and the recover is quick,it is worthy of wide application.
