全程护理在胸腔置管引流治疗恶性胸水中的临床观察

Observe the Whole Care of Chest Tube Drainage in the Treatment of Malignant Pleural Effusion Process

目的：在胸腔置管引流治疗恶性胸水过程中进行全程护理，为该类疾病的护理提供医学参考。方法：选取笔者所在医院收治的32例恶性肿瘤所致胸腔积液患者，所有患者均行胸腔闭式引流，对胸水性质进行观察及胸腔内药物注射，并全程护理。结果：32例患者均一次置管成功，3例导管阻塞，用肝素稀释液3 ml冲管后再通；3例导管脱落重新置管；1例出现气胸经健侧卧位引流管抽气后治愈。经导管胸腔内注入高聚金葡素，结合原发病的综合治疗，所有患者经3~10 d置管，胸腔积液引流彻底。结论：腹腔引流导管闭式引流是一种安全、可靠、损伤小的方法，临床护理到位可有效避免堵管及导管脱落，缓解患者心理压力，有利疾病恢复，缩短住院时间，值得临床进一步推广和应用。

Objective：In the chest tube drainage in the process of the treatment of malignant pleural effusion in the whole nursing,all patients were given the whole nursing,to care for the disease to provide reference of medicine.Method：32 cases with malignant pleural effusion caused by malignant trumor in author’s hospital were selected,all patients were performed chest closed drainage,to look at the chest water properties and the chest cavity medicine injection. Result：32 patients were first catheter successful,3 cases of catheter obstruction with heparin dilution 3 ml tube recanalization after;3 cases of catheter shedding catheter;1 case with pneumothorax after health side-lying position drainage tube suction cured. Patients with malignant pleural effusion chest cavity through the catheter to inject high poly staphylococci,combined with the comprehensive treatment of the primary disease,all patients by 3 to 10 days of catheterization ,and thorough drainage of pleural effusion. Conclusion：Abdominal cavity drainage catheter closed drainage is a safe,reliable,small damage,the method of clinical nursing in place can effectively avoid blocking tube and catheter removal,ease the patients psychological pressure,good disease recovery,shorter hospital stay, worthy of clinical promotion and application further.
