心理因素及癌症家族史与胃癌关系的病例对照研究

A Case-control Study of Relationship of Psychic Factors and Family History of Cancer with Stomach Cancer

目的研究心理因素及癌症家族史对胃癌发生的影响。方法采用 1∶ 1配对病例 -对照研究方法 ,对 346对胃癌病例和对照进行问卷调查。应用条件 L ogistic回归模型进行单因素分析和多因素分析 ,计算比值比及其95 %可信区间。结果精神创伤及精神压抑为胃癌的危险因素 ,比值比分别为 2 .17和 1.73。父亲及母亲患癌史的比值比分别为 2 .80和 1.95。多因素分析有统计学意义的因素有精神压抑和父亲患癌史 ,比值比分别为 1.92 (95 % CI1.0 9～ 3.35 )和 2 .6 0 (95 % CI1.36～ 4.2 8)。结论心理因素及癌症家族史对胃癌发生有一定的影响。胃癌的发病是环境因素。

Objective To study the effects of psychological factors and family history of cancer on the occurrence of stomach cancer. Methods A 1∶1 matched case control study was performed in Linxian, Henan province. 346 cases matched control were interviewed by questionaire. The conditional Logistic regression model was used to do univariate and multivariate analysis. Results The results showed that risk factors of stomach cancer were psychic trauma and depression ( OR =2 17,1 73 respectively).The odds ratio of father and mother with family cancer history were 2 08,1 95 respectively. Through multivariate analysis, the statistical significant factors include psychic constraint and history of father with cancer, and the odds ratio are 1 92(95%CI 1 09~3 35) and 2 60(95%CI 1 36~4 18) respectively. Conclusion Psychological factors and family history of cancer in a certain extent affected the occurrence of stomach cancer, which was a result of integrated actions of factors among environment, social psychology and hereditary tendency.
