血清尿酸水平与糖尿病前期人群早期动脉粥样硬化的关系 被引量:3

Relationship between serum uric acid level and early atherosclerosis of patients with prediabetes

摘要 目的观察糖尿病前期人群血清尿酸水平及脉搏波传导速度(PWV),并分析尿酸水平与早期动脉粥样硬化的关系。方法选择健康体检者2 252例,根据其血糖水平分为正常血糖组1 629例、空腹血糖调节受损组112例、糖耐量异常组465例、糖调节受损组46例,检测各组血生化指标及PWV。结果空腹血糖调节受损组与糖耐量异常组、糖耐量异常组与糖调节受损组的BMI、腰围、收缩压、空腹胰岛素、胰岛素抵抗指数、甘油三酯、PWV比较差异均有统计学意义(P均<0.01),随着BMI、年龄、空腹血糖、75 g口服葡萄糖耐量试验2 h血糖升高,PWV显著上升,且空腹血糖、75 g口服葡萄糖耐量试验2 h血糖与血清尿酸存在交互作用(P<0.01)。结论血清尿酸与早期动脉粥样硬化相关,对糖尿病前期人群应尽早干预,防止糖尿病大血管病变的发生、发展。 Objective To observe the relationship between serum uric acid level and pulse wave velocity (PWV) of patients with prediabetes, and further to analyze effects of interactions between uric acid and metabolic factors on the early atherosclerosis. Methods A total of 2 252 individuals who were selected from residents living in the urban area of Xuzhou were included in the final analysis, and then they were divided into 4 groups : normal glucose tolerance group ( NGT group, n = 1 629), impaired fasting glucose group (IFG group, n = 112), impaired glucose tolerance group (IGT group, n = 465) and impaired glucose regulation group (IGR group, n =46). Biomarkers and PWV in each group were detected and the statistical analysis was conducted. Results There were significant differences in body mass index (BMI), waist circ- umference, systolic blood pressure, fasting insulin (FINS), homeostasis model assessment of insulin resistance (HOMA- IR), triglyceride (TG) and PWV between IFG group and IGT group, between IGT group and IGR group (all P 〈 0.01 ). With the increase of BMI, age, fasting glucose and 2 h PG (2-hour plasma glucose), PWV was also significantly in- creased, meanwhile, there were interactions between fasting blood glucose, 2 h PG and serum uric acid (P 〈 0.01 ). Con- clusion The serum uric acid is associated with early atherosclerosis, and patients with prediabetes should be intervened early in order to prevent the occurrence and development of diabetic macrovascular disease on them.
