经皮肾镜气压弹道联合超声碎石治疗复杂性肾结石

Percutaneous Nephrolithotomy Combined with Pneumatic and Ultrasonic Lithotripsy for the Patients with Complex Renal Calculi

目的：探讨超声引导下经皮肾镜气压弹道联合超声碎石治疗复杂性肾结石的临床疗效。方法：回顾性分析2008年9月-2011年12月在B超引导下穿刺建立经皮肾镜工作通道，联合气压弹道与超声碎石治疗复杂性肾结石160例患者临床资料。结果：手术时间30-140分钟，单通道取石143例，双通道取石11例，三通道取石6例；结石清除率88．75％（142/160），2次手术取石18例，术中输血2例，术后发热5例，术后延迟出血3例。结论：B超引导经皮肾镜气压弹道联合超声碎石治疗复杂性肾结石安全有效，清石率高，并发症少。

Objectives：To investigate the clinical effect of percutaneous nephrolithotomy （ PCNL） combined with pneumatic and ultrasonic lithotripsy under the guidance of B -mode ultrasound for the patients with complex renal calculi .Methods：We retrospectively analyzed the records of 160 patients with complex renal calculi who underwent PCNL combined with pneumatic and ultrasonic lithotripsy under the guidance of B-mode ultrasound .Results：The operation time was 30 min to 140 min.A single tract was used in 143 patients,while two tracts were created in the 11 patients and three tracts in the 6 patients.In this series,the rate of stone clearance was 88.75%（142/160）. A second procedure to remove the stones was achieved in 18 patients.2 patient were given blood transfusion .5 patients fevered and 3 patients had delayed bleeding postoperation .Conclusion：PCNL combined with pneumatic and ultrasonic lithotripsy under the guidance of B-mode ultrasound is efective ,safe, the high rate of stone clearance and less complication for complex renal calculi .
