A comparative study on ^(99) Tc^m-PPM and ^(99) Tc^m-MIBI imaging in diagnosis of primary lung cancer

摘要 目的 探讨99Tcm 培普利欧霉素 (PPM)及99Tcm 甲氧基异丁基异腈 (MIBI)肺显像诊断原发性肺癌的临床价值。方法 对 2 8例临床诊断肺肿瘤患者进行99Tcm PPM及99Tcm MIBI肺肿瘤显像并进行比较。结果 肺癌与良性病变对99Tcm PPM及99Tcm MIBI的摄取差异有显著性 (P <0 0 1)。以肺癌晚期显像摄取比值 (T/NT , x -s)作为判断良恶性阈值 ,99Tcm PPM及99Tcm MIBI对肺癌诊断的灵敏度、准确性分别为 90 %、89 3%和 80 %、82 1% ,两者结合显像阳性率为 95 %。病灶对放射性药物的摄取与肿瘤大小无明显相关 (P >0 0 5 )。结论 99Tcm PPM及99Tcm MIBI肺显像对肺癌的诊断、鉴别诊断有一定的临床价值 ,两者联合应用可提高肺癌的阳性检出率 ,单独使用99Tcm PPM优于99Tcm MIBI。 Objective To evaluate the clinical value of 99 Tc m pepleomycin(PPM) and 99 Tc m methoxyisobutylisonitrile (MIBI) imaging for the diagnosis of lung neoplasms Methods The radionuclide imagings were performed on 28 patients with pulmonary lesions The uptake ratio(T/NT), including early ratio and delayed ratio, was calculated Results The uptake ratios(T/NT) of 99 Tc m PPM or 99 Tc m MIBI were different between malignant and benign lesions( P <0 01) When the delayed radio(T/NT, -s ) was regarded as the threshold, the sensitivity and overall accuracy of 99 Tc m PPM and 99 Tc m MIBI imaging for the malignant lesion were 90%,89 3% and 80%,82 1%, respectively, the positive rate of combined imaging was 95% There were no significant differences between the uptake ratios (T/NT) of different size of malignant tumors( P >0 05) Conclusion 99 Tc m PPM and 99 Tc m MIBI imaging are useful in differentiating malignant lung neoplasms from benign ones and the combined imaging with 99 Tc m PPM and 99 Tc m MIBI can increase the positive rate of detecting lung cancer
