摘要
目的 探讨99Tcm 抗粒细胞单抗BW2 5 0 / 183骨髓免疫显像诊断恶性肿瘤骨转移的临床价值。方法 14例疑有恶性肿瘤骨转移的患者接受了99Tcm BW2 5 0 / 183骨髓免疫显像和99Tcm 亚甲基二膦酸盐 (MDP)骨显像 ,对比分析两种检查结果 ,并与X线和 (或 )MRI比较。结果 骨髓免疫显像在 4例骨转移患者中新发现骨显像未发现的病变部位 8个 ;6例骨转移患者存在外周骨髓扩张 ;骨髓免疫显像和骨显像诊断骨转移的灵敏度分别为 90 %和 10 0 % ,特异性分别为 10 0 %和 2 5 % ,准确性分别为 92 9%和 78 5 %。结论 99Tcm BW2 5 0 / 183骨髓免疫显像诊断恶性肿瘤骨转移有较高的灵敏度和特异性 ,对于骨骼孤立病灶的良恶性鉴别有重要的临床价值。
Objective To study the clinical value of bone marrow immunoscintigraphy in determination of bone metastasis Methods 14 patients with suspected bone metastases underwent bone marrow immunoscintigraphy and conventional bone scan to detect bone metastases The two modalities were completed within the same week using the same technologic condition The results were compared with those obtained with X ray,CT or MRI Results In 10 patients with bone metastases,4 were found to have 8 new bone metastasis foci by bone marrow immunoscintigraphy, and expanded hematopoietic marrow was defined in 6 patients The sensitivity and specificity of bone marrow immunoscintigraphy and of conventional bone scan for determining the presence or absence of bone metastasis were 90%,100% and 100%,25%,respectively Conclusion Bone marrow immunoscintigraphy using 99 Tc m BW250/183 is of fairly high sensitivity and specificity in detecting bone metastases
