基质金属蛋白酶9在肾细胞癌中的表达与患者临床预后的相关性研究

摘要 目的检测基质金属蛋白酶9(MMP-9)在肾细胞癌中的表达,探讨其与患者临床预后的相关性。方法肾癌患者150例,另设癌旁组30例和健康对照组20例,利用免疫组织化学染色检测肾癌组织,癌旁组织及正常肾脏组织中MMP-9的表达。并联系相关随访记录。探讨两者之间的相关性。结果 (1)MMP-9在肾癌组织中高表达,肾肿瘤旁组织及正常肾脏组织中低表达,肾癌组与肾肿瘤旁组织及正常肾脏组织分别比较,均有显著差异(P<0.05)。(2)MMP-9表达与肾癌病人的年龄,肿瘤大小没有相关性(P=0.412;P=0.083),与肿瘤分期、分级及淋巴结转移成正相关(P=0.009;P=0.017;P=0.004)。伴有MMP-9高表达的肾癌患者无复发生存率明显低于MMP-9低表达的患者(P<0.001)。结论 MMP-9在肾癌组织中呈持续高表达,与肾癌患者的临床病理指标,生物学行为和预后密切相关,是评估肾癌进展,不良预后的重要指标。 Objective:To investigate the expression of matrix metalloproteinase-9( MMP-9 )in renal cell carcinoma and determine its clinical significances. Methods:The samples were divided into 3 groups:renal cell carcinoma group(150 cases),paracarcinoma group(30 cases)and normal kidney group(20 cases). The expression of MMP-9 in renal cell carcinoma tissues,paracarcinoma tissues and normal kidney tissues were detected by immunohistochemistry. Results:The expression of MMP-9 proteins in renal cell carcinoma tissues were significantly stronger than paracarcinoma tissues and normal kidney tissues,and the differences were significant(P﹤0. 05). The expression of MMP-9 in renal cell carcinoma tissues was positively correlated with tumor stage,grade and lymph node metastasis(P=0. 009;P=0. 017;P=0. 004). The expression of MMP-9 had no relation with the patients’age and tumor size(P=0. 412;P=0. 083). The patients with high expression of MMP-9 had a significantly lower recurrence-free survival rate than patients with low expression of MMP-9(P﹤0. 001). Conclusions:The expression of MMP-9 is closely correlated with clinical characteristics,biologi-cal behaviors and prognosis. The detection of MMP-9 can serve as an important marker for early diagnosis,development and prognosis of renal cell carcinoma.
