探讨人工膝关节置换术后早期无痛性康复的效果

目的观察人工膝关节置换术后早期无痛性康复的临床效果。方法选取55例患者（90膝）为本次研究对象，随机分为2组，对照组27例（46膝）和观察组28例（44膝）。对照组患者术后均不接受镇痛处理，并按照常规模式展开关节功能训练，持续2～3周；观察组则在上述基础上接受镇痛护理。术后对2组患者无痛性恢复效果进行观察和评估，评分标准参照美国特种外科医院膝关节评分标准（HSS）。结果对照组5例行走正常，观察组16例行走正常；对照组3例关节稳定性正常，观察组51例关节稳定性正常；对照组4例肌力优，观察组28例肌力优。与对照组患者相比，观察组疼痛程度更低、关节稳定性、活动度、患肢肌力以及行走功能均明显更优（P〈0.05）。结论接受人工膝关节置换术的患者实施早期无痛康复可有效缓解疼痛，并有利于术后患者关节功能的顺利恢复，应在临床推广应用。

Objective To observe artificial knee joint replacement painless early clinical results of rehabilitation. Methods 55 cases （a total of 90 knees） were randomly divided into control group 27 patients （46 knees） and observation group 28 patients （44 knees）. The control group didn＇t received analgesic treatment and done the joint functional training according to the convientional, for 2 to 3 weeks. The observation group was accepted analgesic treatments based on the abobve treatment. The recovery effect of the two groups was obserbved and evaluated. Scoring criteria references American Hospital for Special Surgery knee score （HSS）. Results 5 cases in the control group could walk normally, 16 cases in the observation group could walk normally;3 cases in the control group of normal joint stability, 31 cases in the observation group of normal joint stability ;4 cases in the control group was superior strength, 28 cases in the observation group was superior strength. Compared with the control group, a lower degree of pain observation group, joint stability, activity, limb muscle strength and walking function were significantly better （P〈0.05）. Conclusion Patients with artificial knee joint replacement accepted the implementation of early rehabilitation can relieve pain, and contribute to a smooth recovery postoperative ioint function should be in clinical application.
