超声造影肝实质灌注峰值强度评价肝硬化的可行性研究 被引量:4

Evaluation on the practibility of peak intensity of liver parenchyma during perfusion in patients with of cirrhosis by contrast-enhanced ultrasonography

摘要 目的探讨超声造影肝实质灌注峰值强度反映肝硬化肝实质的血流灌注变化的可行性。方法 35例肝硬化患者(病例组),21例健康人(对照组)进行超声造影检查,应用QLAB分析软件,将肝实质作为感兴趣区绘制时间-强度曲线,测量肝实质灌注峰值强度(Ipeak)。比较两组间Ipeak有无差异,并应用受试者操作特性(ROC)曲线进行分析。结果病例组与对照组比较,病例组的Ipeak小于正常组(P<0.05)。当Ipeak诊断肝硬化的界值为30.20 dB时,灵敏度为90.4%,特异度为74.8%。结论肝硬化患者和正常人之间超声造影Ipeak存在显著差异。Ipeak可以反映肝硬化肝脏血流灌注的变化,对判断有无肝硬化有着较高的灵敏度和特异度。 Objective To evaluate the pracfibility of peffusion assessed by contrast - enhanced ultrasouography (CEUS) for accurately and non - invassively predicting cirrhosis. Methods Thirty - five patients with cirrhosis and twenty - one healthy controls were given with CEUS examination, and time - intensity curves (TIC) were drawn as the regions of interest localities in liver parenchyma by using QLAB analyzing soft ware. The parameters of these two groups were as follows : peak intensity of liver parenchyma (Ipeak) by independent - samples t test, and the diagnostic value of parameters were analyzed by receiver operating characteristic (ROC) curve. Results Ipeak was smaller in cirrhosis patients than that of controls, the difference was statistically significant. The critical value of Ipeak was 30.20 and when it used for diagnosis of cirrhosis, its sensitivity reached 90.4% and its specificity was 74.8%. Conclusion CEUS can reflect the changes in blood perfusion of liver in cirrhotic patients. The parameter Ipeak of CEUS is significantly different between these two groups and it can be used as non - invasive parameter for diagnosis of cirrhosis.
