微创两步截冠法与去骨劈冠拔除法拔除水平阻生齿的对比研究 被引量:10

摘要 目的比较微创两步截冠法与去骨劈冠拔除法拔除水平阻生齿的术后反应,验证微创拔牙法的优越性。方法选2013年1月至2013年12月需要拔除水平阻生齿的患者93例,随机分为观察组47例和对照组46例。观察组采用微创两步截冠法,对照组则常规去骨劈冠后拔除。比较两组拔牙成功率、手术时间、术后局部疼痛、肿胀情况。结果应用微创两步截冠法的拔牙成功率为95.74%,对照组拔牙成功率100%,但差异无统计学意义(P>0.05)。观察组手术时间(19.95±5.26 min)短于对照组(31.74±6.83 min)(P<0.05)。观察组术后1 d、3 d基本没有发生肿胀的患者比率(53.19%,74.47%)均比对照组(21.74%,39.13%)高(P<0.05);观察组术后1 d、3 d基本无痛的患者比率(38.30%,59.57%)均比对照组(10.87%,32.61%)高(P<0.05)。结论采用微创两步截冠法拔除水平阻生齿能够减少患者术后创伤反应,减轻患者疼痛,提高患者生活质量。 Objective To compare the postoperative reaction of minimally -invasive two step extraction and traditional boneless split crown extraction for horizontal impacted tooth, so as to further verify the advantages of minimally invasive extraction. Methods A total of 93 patients with horizontal impacted tooth during January to December 2013 in this hospital were randomly divided into observation group (43 cases) and control group (42 cases). Teeth in patients of observation group were extracted by minimally invasive two steps crown truncation method. Teeth in patients of control group were extracted by conventional boneless split crown method. Local pain, swelling after operation were compared between these two groups. Results The successful rate of minimally invasive two step extraction for horizontal impacted tooth was 95.74% in observation group was shorter than that of control group 100%. But the difference was not statistically significant( P 〉 0.05 ). The operating time in observation group ( 19.95 ± 5.26 min)was shorter than that of control group (31.74 ± 6.83 min) ( P 〈 0.05 ). The degree of pain and swelling in observation group was significantly lower than that of control group ( P 〈 0.05 ). Conclusion Minimally - invasive two step extraction for horizontal impacted tooth can reduce the reaction of postoperative wound.
