缺铁性贫血人群全血糖化血红蛋白含量水平分析及参考区间建立 被引量:7

摘要 目的了解深圳龙华新区缺铁性贫血人群全血糖化血红蛋白百分含量(HbA1c)水平,并建立其参考区间,更好地为临床病尿病的诊断、治疗及疗效判断提供有效的科学依据。方法随机收集龙华新区2012年10月至2013年9月期间血糖在4~6mmol/L之间缺铁性贫血人群120例和健康人群80例,并确保各比较组血糖浓度分布一致。分别用高效液相色谱法(HPLC)测定全血中HbA1c含量,并对所测数据分别按组别进行统计分析。结果缺血性贫血人群和健康正常人群全血中的HbA1c含量分别为(4.83±3.26)%和(4.35±2.51)%,缺血性贫血人群高于健康正常人群,差异有统计学意义(t=5.2547,P〈0.05);男女缺铁性贫血人群全血中的HbA1c含量分别为(4.89±2.65)%和(5.01±3.13)%,女性缺铁性贫血人群略高于男性,男女差异无统计学意义(t=1.9264,P〉0.05);缺铁性贫血人群HbA1c含量随着贫血严重程度增加而增高,不同组别之间差异有统计学意义(t=4.8154,P〈0.05);缺铁性贫血人群和健康正常人群全血中HbA1c参考区间分别为(4.55~6.82)%和(4.21—6.13)%,缺血性贫血人群高于健康正常人群,两者差异有统计学意义(t=3.9257,P〈0.05)。结论缺铁性贫血人群HbA1c水平高于健康正常人群,各地应分别建立相应的生物参考区间,更好地为各类人群相应疾病的诊断、治疗及疗效观察提供更科学可靠的依据。 Objective To understand whole blood glycated hemoglobin(HbAlc) content levels in population of irondeficiency anemia in the Shenzhen Longhua new district, and to establish the reference range, which may be helpful in the diabetes clinical diagnosis, treatment and curative effect judgment. All these results can provide effective scientific basis. Methods 120 iron deficiency anemia cases and 80 cases of healthy crowd were collected randomly in the Longhua new district from October 2012 to September 2013 during the blood sugar in 4 - 6 mmol/L. We should make sure that blood sugar concentration distribution of each comparison group is consistent. High performance liquid chromatography (HPLC) method was developed for the determination of HbAlc in whole blood levels. The measured data were analyzed according to the different group. Results The HbAlc levels in the blood in iron deficiency anemia crowd and healthy normal were (4.83 ±3.26 ) % and (4.35± 2. 51 ) %, respectively. The level of iron deficiency anemia crowd is higher than that of normal healthy people. The different results between the two group was statistically significant ( t = 5. 2547, P 〈0.05 ). The HbAlc levels in the blood in meng and womeng iron deficiency anemia crowd were (4.89 ±2.65 ) % and ( 5.01 ±3.13 ) %. The HbAI c in female iron - deficiency anemia is slightly higher than that of the male crowd. There was no statistically significant difference between the results ( t = 1. 9264, P 〉 0.05 ). Iron deficiency anemia crowd HbAlc content increases with the increasing of the severity of anemia. The differences between different groups were statistically significant ( t = 4. 8154, P 〈 0.05 ). The reference interval in the whole blood in iron deficiency anemia and healthy crowd normal HbA1 c were (4.55± 6.82)% and (4.21 -6.13 )%. The level of HbAlc in the iron deficiency anemia crowd is higher than that of normal healthy crowd. The difference between the two groups was statistlcally significant difference ( t = 3. 9257, P 〈 0.05). Conclusion Iron deficiency anemia crowd HbAlc levels were higher than that of normsl healthy people. Everywhere should set up a corresponding biological reference range, which is better for all kinds of people to corresponding disease diagnosis, treatment, curative effect observation and provide a more scientific and reliable basis.
