糖尿病视网膜病变激光治疗技术标准探讨

Study of the technique criteria of the laser treatment for diabetic retinopathy

探讨糖尿病视网膜病变激光治疗技术标准及疗效。方法 :根据 DRPSG( Diabetic Retinopathy Photocoagulation StudyGroup)制订的治疗技术规定 ,对 70例 10 5眼分别为增殖前期糖尿病视网膜病变 ( PPDR)、增殖期糖尿病视网膜病变 ( PDR)及糖尿病性黄斑水肿患者行激光治疗 ,按技术规定的光凝范围和光凝参数行标准全视网膜光凝 ( S- PRP)和超全视网膜光凝 ( E-PRP)及黄斑水肿行局限光凝或格栅光凝 ,激光后 3个月、 6个月、 12个月行 FFA及彩色眼底像 ,新生血管未消退者和无灌注区尚存者追加光凝 ,随访 3～ 12个月 (平均 8.91个月 )。结果 :2 0眼弥漫性黄斑水肿格栅光凝后 ,13眼 (约 6 5 .0 % )水肿减轻或消失 ,10 2眼行全视网膜光凝 ,新生血管部分或全部消退 73眼 (约 71.6 % ) ,10 5眼中视力不变和增进 81眼 (约 77.1% )。结论 :按DRPSG制订的治疗技术规定治疗糖尿病视网膜病变是行之有效的 。

Purpose:To probe the technique criteria and effects of the laser treatment for diabetic retinopathy.Methods:According to the treatment technique stipulations formulated by DRPSG(Diabetic Retinopathy Photocoagulation study Group),105 eyes in 70 cases respectively suffering from preproliferrative diabetic retinopathy (PPDR),proliferative diabetic retinopahy (PDR) and diabetic macular edema were treated with laser treatment.These patients were received the treatment of standard panretinal photocoagulation (S PRP) and extra panretinal photocoagulation (E PRP).Those with macular edema took the focal and grid photocoagulation.All these treatment were performed with the range and parameters provided by the treatment technique stipulations.The Fundus Fluorescein Angiography (FFA) and color photograph were taken 3m,6m and 12m after the treatment.For those cases with remaining new vessls and non perfusion areas,the treatment was complemented with further photocoagulation.The follow up period was 3 12 months (with an average of 8 91 months).Results:After laser grid photocoagulation in 20 eyes with diffuse edema,edema in 13 eyes (about 65 0%) was found considerably improved or disappeared.Retinal new vessels partly or entirely regressed in 73 eyes (about 71 6%) of the 102 eyes subjected to PRP.Visual acuity were improved or maintained in 81 eyes (about 77 1%) of 105 eyes.Conclusions:Its of significant effect to treat diabetic retinopathy according to treatment technique stipulations formulated by DRPSG.Following the stipulations laser treatment for diabetic retinopathy is more standard.
