Detection and analysis of beta-lactams resistance related genes in multidrug-resistant Shigella

摘要 目的了解多重耐药志贺菌β-内酰胺类耐药相关基因检测情况,为治疗提供依据。方法收集嘉兴地区2006年11月-2012年10月间多重耐药志贺菌,以聚合酶链反应(PCR)法检测β-内酰胺类基因、接合性质粒、整合子、转座酶、插入序列和复方新诺明类基因共14种;β-内酰胺类基因和可移动元件遗传元件存在状况的相关性用指标聚类分析(SPSS法)。结果 58株多重耐药志贺菌检出β-内酰胺类基因3种(CTX-M-1、TEM、OXA),可移动遗传元件标记基因检出5种(traA、trbC、intI 1、ISEcp1、IS903)。结论本组志贺菌β-内酰胺类抗菌素耐药与携带β-内酰胺类耐药基因相关。可移动遗传元件是致本组志贺菌多重耐药的主要原因。指标聚类分析提示可移动遗传元件和获得性耐药相关基因的携带及转移密切相关。 Objective To investigate the present situation of beta - lactams genes in multidrug - resistant Shigella, so as to pro- vide basis for treatment of multidrug - resistant Shigella infection. Methods Multidrug - resistant Shigella isolates from Jiaxing during November 2006 and October 2012 were collected to detect 14 kinds of resistance genes including beta -lactams gene, conjugation plasmid, integrons, transposons, insertion sequences and sulfamethoxazole - trimethoprim resistance genes using PCR. Index cluster analysis (SPSS method) was used to analyze the relationship between beta- lactams gene and mobile genet- ic elements. Results Three kinds of beta - lactams genes ( CTX - M - 1, TEM, OXA), and 5 marker genes of mobile genetic elements(traA, trbC, intI 1; ISEepl, IS903) were detected in 58 multidrug- resistant Shigella isolates. Conclusion ShigeUa containing the beta - lactams resistance genes were resistant to antimierobial agents of beta - lactams. Mobile genetic elements are the main reason of multidrug - resistant Shigella. Index cluster analysis suggests that mobile genetic elements are closely re- lated to the presence and transfer of acquired resistance - related genes.
