白内障超声乳化术对高度近视眼角膜内皮的影响

The influence of phacoemulsification on corneal endothelium of high myopia

目的 :探讨白内障超声乳化术对高度近视眼角膜内皮的影响。方法 :采用上方弧形 5 .5 cm巩膜隧道切口行白内障超声乳化术。用非接触式角膜内皮显微镜分别于术前、术后 1月拍摄正常组 30例 ( 5 0眼 )与高度近视眼组 2 4( 48眼 )角膜内皮细胞图像并行计算机图像处理及自动分析。结果 :各组中术前与术后细胞密度、细胞面积、变异系数、六角形细胞比例均有显著性差异 ( p<0 .0 5 ) ;两组间术前与术后细胞密度、细胞面积、细胞丧失率 ,六角形细胞比例经统计学分析有显著性差异 ( p<0 .0 5 ) ,同时两组年龄统计学无显著性差异 ( p>0 .0 5 )。结论 :高度近视眼在白内障手术中其内皮细胞比同龄正常眼更易受到损伤 ,临床上应该慎重手术。

Objective:To study the influence of phacoemulsification on corneal endothelium of high myopia.Methods:54 patients underwent phacoemulsifcation and intraocular lens implantation through a upper sclera tunnel incision.The endothelial cells in the high myopia group and the control group were analyzed before and after 1 month of operation.Results:After operation,there were obvious differences in cell area,cell density,the rate of hexagonality in each group(p<0 05);There were also obvious differences between two groups before and after 1 month of operation;In the same time,no differences in ages between two groups.Conclusions:The endothelial cells of high myopia are easier to be damaged than that of senile cataract in the phacoemulsification,So safe and reliable operation should be selected for these patients.
