减压阀引流和睫状体冷冻术治疗新生血管性青光眼疗效分析

Glaucoma valve devices and cyclocryotherapy for neovascular glaucoma

目的 :比较减压阀引流术和睫状体冷冻术治疗新生血管性青光眼的疗效和并发症。方法 :将 1993年 1月至 1998年 9月间分别施行这两种术式的年龄和病因基本匹配的两组患者进行比较 ,其中减压阀引流术组 32例 ( 32眼 ) ,睫状体冷冻术组 2 9例 ( 2 9眼 )。结果 :减压阀引流术组出院时成功率为 84.4% ,平均随访 2 4个月 ,成功率为 5 2 .6 %。睫状体冷冻术组出院时成功率为 2 7.6 % ,平均随访 32个月 ,成功率为 47.1%。两组远期效果比较无显著性差异 ( P>0 .0 5 )。减压阀引流术组未见严重并发症 ,睫状体冷冻术组有 2眼发生眼球萎缩。结论 :减压阀引流术能较安全、有效的治疗新生血管性青光眼 ,特别是存有视功能的患者 ,睫状体冷冻术存在严重的并发症 。

Purpose:TO compare the effectiveness and complication of glaucoma Valve devices and cyclocryotherapy in the treatment of neovascular glaucoma.Methods:Patients with neovascular glaucoma having either of the two procedures from Jane,1993 ～ sept,1998 were selected.Thirty two cases,(32 eyes)underwent glaucoma valve devices for filtration surgery,while twenty nine case,(29 eyes)underwent cyclocryotherapy.Results:the success control rates with glaucoma valve devices were 84 4% at the time of discharge from hospital and 52 6% at the time of mean 24 months follow up;while the rate with cyclocryotherapy were 27 6% at the time of discharge from hospital and 47 1% at the time of mean 32 months follow up.There were no severe complication with glaucoma value devices while two cases with cyclocryotherapy had atrophy of eye ball.Conclusions:the glaucoma valve devices is in wore efficacious and safer than cyclocryotherapy for neovascular glaucoma.
