门控心肌灌注显像判断疑似病毒性心肌炎的临床价值

摘要 目的探讨门控心肌灌注显像对疑似病毒性心肌炎诊断的价值。方法应用99 m锝-甲氧基异丁基异晴(99mTc-MIBI)对62例疑似病毒性心肌炎患者进行门控心肌灌注显像,观察心肌血流灌注情况,了解患者心功能变化情况。结果 62例疑似病毒性心肌炎血流灌注异常47例,阳性率75.8%,病变心肌表现为花斑样放射性稀疏影,以单病灶损害为主,常见损害部位为:前壁及下后壁。心功能异常19例,主要是运动协调性降低及舒张功能降低。结论门控心肌灌注显像是一种无创性检查方法,能直观显示心肌病变的部位、范围和严重程度,获得心脏功能参数,对疑似病毒性心肌炎诊断有重要临床价值,可作为诊断病毒性心肌炎的一种重要辅助方法。 Objective To investigate the diagnostic value of gated myocardial perfusion imaging in suspected viral myocarditis. Methods ^99mTc - MIBI was applied to do gated myocardial perfusion imaging for 62 patients with suspected viral myocarditis, observed the myocardial perfusion and cardiac function changes in patients.. Results 47 in 62 eases of suspected viral myocarditis performed perfusion abnormality, the positive rate was75.8%, myocardial lesions manifested as piebald radioactive dilute Shuying and most were single lesion, mainly present at the front wall and the rear wall. Abnormal cardiac function occurred in 19 cases, mainly were decreasing motion coordination and diastolic function. Conclusions Gated myocardial perfusion imaging is a noninvasive method, which could intuitionally reveal the location, morpha, range and degree of pathological myocardium, it has important clinical value to the diagnosis of viral myocarditis. Meanwhile, it could be used as a kind of important auxiliary diagnosis of viral myocarditis.
