剖宫产术中缝合止血方法分析

Analysis of the stitching hemostasis in uterine- incision delivery

目的通过分析我院所采用的几种剖宫产术中的缝合止血方法,总结各种止血缝合术的操作方法、适用范围、止血效果、术后并发症。方法分析我院产科在2012年1月至2013年12月间对剖宫产术中出血所采用的4种方法缝合子宫(宫腔内"8"字缝合、宫腔外大"8"字缝合、改良B-Lynch子宫缝合术、子宫动脉结扎术)的所有病例。结果以上几种缝合子宫止血术安全、效果良好、操作简单,但分别适合于不同情况下的止血要求,临床应用时应正确选择、以有确切效果且尽量减少损伤为原则。结论在产科临床工作中,灵活掌握并运用剖宫产术中多种缝合止血方法,对降低剖宫产术中及术后出血、降低产科DIC及子宫切除发生率有十分重要意义。

Objective To summarize the operation method, scope of application, effect of hemostasis and postoperative complications of the hemostasis methods we used in practical work. Methods Retrospectively analyze the bleeding cases those who received uterine - incision delivery stitching （ intrauterine ＂ 8＂ suture, outside big ＂8＂ suture of uterus, improved B - Lynch uterine suture technique, uterine artery ligation） in our hospital during January 2012 and December 2013. Results All the suture methods were safe, effective and easy to operate, but they suitable for different cases of bleeding, we should correctly choose to ensure the efficacy and minimize the damage in clinical application. Conclusions Mastering and using suture methods smoothly would helpful for decreasing intra-/post-operative bleeding, DIC and hysterectomy.
