促宫颈成熟药物对延期妊娠孕妇分娩结局及围产儿的影响

Effect of cervical ripening drugs on birth outcomes and perinates of women with delayed pregnancy

目的探讨促宫颈成熟药物对延期妊娠孕妇分娩结局及围产儿的影响。方法回顾性分析2010年3月至2013年6月我院收治的137例延期妊娠孕妇,其中45例应用普贝生引产(A组),45例应用米索前列醇引产(B组),47例应用催产素引产(C组)。比较三组促宫颈成熟效果及分娩结局。结果 A组促宫颈成熟有效率为91.11%,均显著高于B组和C组的71.11%和61.70%,差异有统计学意义(P<0.05)。A组引产成功38例,成功率为84.44%,自然分娩率为77.78%,均显著高于B组(26例,57.78%,46.675%)和C组(28例,59.57%,29.78%),差异均有统计学意义(P<0.05);但三组间胎儿宫内窘迫和新生儿窒息发生率比较则差异均无统计学意义(P>0.05)。不良反应方面三组间比较差异亦无统计学意义(P>0.05)。结论与米索前列醇和催产素比较,普贝生能够起到较好的促宫颈成熟效果,提高引产成功率和自然分娩率,且对孕妇和产儿无不良影响,是较为有效的促宫颈成熟药物。

Objective To discuss the effect of cervical ripening drugs on birth outcomes and perinates of women with delayed pregnancy. Methods The clinical data of 137 women with delayed pregnancy from March 2010 to June 2013 were retrospectively analyzed, of which 45 women used dinoprostone for induced labor （group A）, 45 used misoprostol for induced labor （group B）, and 47 used oxytocin for induced labor （group C）. The cervical ripen-ing effect and birth outcomes were compared between the three groups. Results The effective rate of cervical ripen-ing was 91.11%in group A, significantly higher than that in group B （71.11%） and group C （61.70%）, P〈0.05. 38 women in group A were performed induced labor successfully, with the success rate of 84.44%, natural birth rate of 77.78%, signifi-cantly higher than those of group B （26, 57.78%, 57.78%） and group C （28, 59.57%, 29.78%）, P〈0.05. The differences of fetal distress and neonatal asphyxia between the three groups were not significant （P〉0.05）. And the differences of adverse reactions between the three groups showed no statistically significant differece （P〉0.05）. Conclusion Compared with misoprosto and oxytocin, dinoprostone has better effect on promoting cervical mature and improving the success rate of induced labor and natural birth, and results in no adverse effects on pregnant women and babies, which serves as a more effective cervical ripening drug.
