PPH联合皮瓣弧形切除缝合术治疗重度环状混合痔疗效观察

The clinical efficacy observation of 32 cases with severe circumferential mixed hemorrhoids treated by PPH combined with flap curving suture technique

目的探讨PPH联合皮瓣弧形切除缝合术治疗重度环状混合痔的临床疗效。方法将64例患者随机分为治疗组(PPH联合皮瓣弧形切除缝合术)和对照组(传统外剥内扎术),每组32例。比较两组住院时间、术后疼痛评分、术后水肿发生率、术后肛缘皮赘残留率的情况。结果治疗组在住院时间、术后疼痛评分、术后水肿发生率、术后肛缘皮赘残留率方面均优于对照组,差异具有统计学意义(P<0.05)。结论 PPH联合皮瓣弧形切除缝合术不但解决了外痔部分皮赘残留的问题,还尽可能的保留了肛管上皮,保护了肛门的正常生理结构和功能,使PPH的优越性得到进一步体现。

Objective To approach comparative of curing severe circumferential mixed hemorrhoids by procedure for prolapsed hemorrhoid （PPH） combined with flap curving suture technique and open Milligan Morgan hemorrhoidectomy. Methods To analyze clinical data from 64 cases of hemorrhoids patients, which were divided into open Milligan Morgan hemorrhoidectomy group （N= 32 ） and PPH combined with flap curving suture technique group （N=32）. The hospitalization time, postoperative pain score, the incidence of anal swelling, and the retention rate of anal skin tags were compared between two groups. Results The hospitalization time, postoperative pain score, the incidence of anal swelling and retention rate of anal skin tags in patients undergoing PPH combined with the flap curving suture technique group were lower than open those with Milligan Morgan hemorrhoidectomy group（ P 〈0.05）, which had statistical significance difference. Conclusion PPH combined with the flap curving suture technique could not only solve the prob- lem of retention rate of anal skin tags,but aslo keep the anal canal epithelium as far as possible,and protect the normal physiological structure and function of the anus.
