水平和后半规管类型良性阵发性位置性眩晕动态平衡功能的比较 被引量:3

摘要 目的探讨水平和后半规管类型良性阵发性位置性眩晕(BPPV)患者动态平衡功能的特点,明确动态姿势图检查的意义及必要性。方法通过对本院眩晕专科门诊44例后半规管BPPV(PSC-BPPV)、22例水平半规管BPPV(HSC-BPPV)患者及44例匹配年龄、性别的健康志愿者的动态平衡板试验,获得动态姿态平衡图及感觉结构测试结果。结果感觉结构分析:HSC-BPPV组姿势稳定性较PSC-BPPV组差(P<0.05)。PSC-BPPV组及HSC-BPPV组动态姿势稳定性较对照组均差,差异有显著统计学意义(P<0.01)。结论 PSCBPPV组及HSC-BPPV组动态姿势稳定性较对照组均表现出不同程度的下降。HSC-BPPV组感觉结构测试结果较PSC-BPPV组差。在去除本体觉和视觉的参与下,前庭觉维持平衡的能力明显下降。 Objective To investigate the dynamic equilibrium function in patients with horizontal or posterior semicircular canal benign paroxysmal vertigo (BPPV) and to predict the significance of such tests in the clinical practice. Methods Fourty-four cases of posterior semicircular canal (PSC-BPPV) , 22 eases of horizontal semicircular tube inside (HSC-BPPV) and 44 healthy volunteers matched by age and gender accepted the dynamic balance panel test. The dynamic posture equilibrium diagram (Stabilogram) and sensory organization testing (SOT) were analyzed. Results SOT showed that the postural stability of HSC-BPPV group was worse than PSC-BPPV group ( P 〈 0.05 ). The dynamic postural stability of both the PSC-BPPV group and the HSC-BPPV group was worse than that of the control group ( P 〈 0.01 ). Conclusions The dynamic postural stability of PSC-BPPV and HSC-BPPV group exhibited varying degrees of decline. SOT result of HSC-BPPV group was worse than that of PSC-BPPV group. In removing the proprioceptive and visual participation, the ability of vestibular to maintain balance decreased.
