摘要 目的:探讨DWI联合M RI常规序列对早期子宫内膜癌术前评估的应用价值。方法回顾性分析43例经手术病理证实的I~Ⅱ期子宫内膜癌的M RI资料,观察肿瘤信号、侵及范围,测量肿瘤实质部分的ADC值,并进行肌层浸润深度评估。结果与正常子宫内膜相比,肿瘤T1 WI多呈等信号,T2 WI呈高信号,DWI为明显高信号,ADC值降低。Ia、Ib及II期肿瘤实质部分ADC值比较,差异无统计学意义。M RI常规序列及DWI联合M RI常规序列判断肌层浸润深度的准确率分别为81.4%和86.0%。结论 DWI联合M RI常规序列对早期子宫内膜癌术前判断肌层浸润深度较准确,可作为子宫内膜癌术前评估的重要方法。 Objective To investigate the value of diffusion weighted imaging (DWI) and conventional magnetic resonance imaging (M RI) sequences in preoperative evaluation of endometrial carcinoma at early stage .Methods T he M RI data of 43 cases of pathologically confirmed I~ Ⅱ endometrial cancer were retrospectively analyzed .The signal intensity ,invasion and extent of the tumor were observed ,and ADC value was measured .Then ,we evaluated the depth of myometrial in-volvement of the tumor .Results Compared with normal endometrium ,the tumor was usually isointensity on T1WI ,hy-perintensity on T2WI and DWI ,and lower ADC values .The ADC value of the Ia ,Ib and II stage tumors showed no statis-tically different .The accuracy of conventional MRI sequences and DWI combined conventional MRI sequences was 81 .4%and 86 .0% in assessing deep myometrial invasion .Conclusion DWI combined conventional M RI sequences is accurate for assessing the depth of myometrial invasion of early endometrial cancer .And it can be used as an important method of pre-operative assessment of endometrial cancer .
