肠系膜上动脉压迫综合征诊治分析

ANALYSIS OF THE DIAGNOSIS AND THE TREATMENT OF SUPERIOR MESENTERIC ARTERY SYNDROME

目的 报道肠系膜上动脉压迫综合征的诊断和治疗原则。方法 结合文献回顾性分析我院收治的 1 1例肠系膜上动脉压迫综合征的临床特点。结果 1 1例中 8例首诊者均经上消化道钡餐确诊 ,余 3例经外院手术诊断。单纯保守治疗 5例中痊愈 3例。保守治疗无效而行手术的 6例中痊愈 4例。结论 肠系膜上动脉压迫综合征的诊断目前主要依靠上消化道钡餐检查 ,腹腔血管彩超及动态薄层CT也有助于诊断 ;保守治疗是首选的治疗方法 。

Objective To report the diagnostic and treatment principles for superior mesenteric artery (SMA) syndrome.Methods 11 cases of SMA syndrome treated in our hospital were analyzed retrospectively in combination with the study of the literature concerned.Results 8 cases were diagnosed by hypotonic duodenography, and the other 3 cases were diagnosed through operation.5 patients received conservative therapy only and 3 of them were cured, while the other 6 patients resorted to operation since conservative therapy failed to give response and 4 of them were cured.Conclusion The diagnosis of SMA syndrome should mainly depend on hypotonic duodenography in addition to ultrasonography and CT which are also helpful. Conservative therapy is the first choice of treatment, while operation should be applied in consideration with its indications.
