益肺宣肺降浊胶囊治疗血管性痴呆患者107例临床观察

Clinical Observation on Yifei Xuanfei Jiangzhuo Capsule for 107 Cases of Vascular Dementia Patients

目的观察益肺宣肺降浊胶囊治疗血管性痴呆的临床疗效并探讨其可能作用机制。方法将209例患者随机分为治疗组(107例)和对照组(102例)。治疗组口服益肺宣肺降浊胶囊,每次5粒(1.75g),每日3次;对照组口服经重新包装后为每5粒含吡拉西坦片0.8g的胶囊,每次5粒,每日3次,两组均观察用药2个月。观察治疗前后两组患者认知功能(HDS)评分、日常生活能力(ADL)评分和血清超氧化物歧化酶(SOD)、丙二醛(MDA)、内皮素(ET)、一氧化氮(NO)水平的变化,治疗后评价临床疗效。结果治疗组临床疗效总有效率为51.40%,对照组总有效率为48.04%,两组差异无统计学意义(P>0.05)。两组患者治疗后ADL、HDS评分均显著改善(P<0.05或P<0.01);治疗组治疗后ADL评分显著低于对照组(P<0.05);两组患者治疗后HDS评分比较差异无统计学意义(P>0.05)。两组患者治疗后血清SOD水平显著上升,MDA、ET、NO水平均显著下降(P<0.05或P<0.01);治疗后治疗组血清SOD水平显著高于对照组、MDA水平显著低于对照组(P<0.05)。结论益肺宣肺降浊胶囊治疗血管性痴呆疗效肯定,其机制可能与其降低患者血清ET和NO含量,调节SOD、MDA的水平有关。

Objective To observe and study the efficacy and mechanism of Yifei Xuanfei Jiangzh （ capsule to supplement the lung, diffuse the lung and downbear the turbid） for vascular dementia （VaD） Totally 209 cases of VaD were randomized into the treatment group with 107 cases and control group with uo Capsule Methods 102 cases.The treatment group was given 1.75 g （5 caps） Yifei Xuanfei Jiangzhuo Capsule three times daily. The control group was given 0. 8 g （5 caps） repackaging capsules contained with Piracetam Tablets three times daily. Both groups were treated for 2 months. The scores of Hasegawa＇s Dementia Scale （HDS） and activities of daily living （ADL） and theserum levels of superoxide dismutase （SOD） , malondialdehyde （MDA） , endothelin （ET） and nitric oxide were observed before and after treatment in both groups. The clinical efficacy was evaluated after treatment The total effective rate of the treatment group and control group was 51.40% and 48.04% respectively, with （NO） Results no significant difference between groups （P 〉 0. 05 ）. The scores of HDS and ADL were significantly improved after treatin both groups （P 〈0. 05 or P 〈0. 01 ）, especially ADL score in the treatment group （P 〈0. 05）. There was no significant difference between groups in the HDS score （P 〉 0. 05 ）. The serum level of SOD was significantly in-creased but the serum levels of MDA, ET and NO were significantly decreased in both groups after treatment （P 〈 0. 05 or P 〈 0. 01 ）, especially SOD and MDA in the treatment group （ P 〈 0. 05 ）. Conclusion Yifei Xuanfei Jiangzhuo Capsule is effective for VaD with an equivalent efficacy of Piracetam Tablets, which may be relevant to decreasing the serum levels of ET and NO and regulating the serum levels of SOD and MDA.
