肠易激综合征中西医结合临床路径的应用研究

Application Research on Integrative Medicine Clinical Pathway for Irritable Bowel Syndrome

目的评价肠易激综合征(IBS)中西医结合临床路径应用于临床的实际效果。方法采用同期队列研究,将83例IBS患者分为路径组41例和非路径组42例,路径组患者在治疗指南的基础上,建立IBS中西医结合临床路径;非路径组患者按常规治疗。比较两组患者中医症状积分、平均住院天数和平均住院总费用。结果两组患者治疗后腹痛、腹胀、腹泻、便秘症状积分与治疗前比较均明显降低(P<0.05或P<0.01);路径组腹胀积分治疗前后差值与非路径组比较,差异有统计学意义(P<0.05)。路径组患者平均住院天数为(5.88±2.38)天,非路径组为(6.83±2.88)天,两组比较差异无统计学意义(P>0.05)。路径组平均住院总费用(5609.32±1863.17)元,明显低于非路径组的(6616.33±2440.68)元(P<0.05)。结论 IBS中西医结合临床路径可有效改善患者临床症状、减少平均住院总费用。

Objective To evaluate the efficacy of integrative medicine clinical pathway for irritable bowel syndrome （IBS）. Methods A cohort study was adopted. Totally 83 cases of IBS were randomized into the pathwaygroup with 41 cases and non-pathway group with 42 cases. The integrative medicine clinical pathway was established in the pathway group according to the treatment guidelines. The non-pathway group was given routine treatment. Therelief of clinical symptoms, average hospital stays and average hospital costs were observed in both groups. Results The scores of abdominal pain, abdominal distention, diarrhea and constipation were significantly decreased after treat-ment in both groups （P 〈 0. 05 or P 〈 0. 01 ）, especially the score of abdominal distention in the pathway group （P 〈 0. 05）. The average hospital stays in the pathway group and non-pathway group were （5.88 ± 2. 38 ） days and（6. 83 ± 2.88 ） days respectively, with no significant difference between groups （ P 〉 0. 05 ）. The average hospital costs in the pathway group and non-pathway group were （5609.32 ± 1863.17） yuan and （6616. 33 ±2440. 68） yuanrespectively, with a significant difference between groups （P 〈 0. 05）. Conclusion The integrative medicine clinical pathway for IBS can effectively relieve symptoms and reduce the average hospital costs.
