针灸治疗高血压病选穴规律文献研究

Literature Research on Acupuncture Point Selection for Hypertension

目的通过文献分析探讨针灸治疗高血压病的临床选穴规律。方法检索中国期刊全文数据库、维普中文期刊数据库、万方学术期刊全文数据库、中国生物医学文献网络版数据库中针灸治疗原发性高血压病的临床研究文献,建立处方数据库,采用关联分析法对针灸治疗高血压病的穴位、配伍、分部、经脉、特定穴的使用情况进行分析。结果共纳入文献285篇,针灸处方285首,涉及腧穴62个,腧穴使用总频次为588。针灸治疗高血压病以太冲、曲池、风池、合谷、足三里、百会为主穴,并随证加减;高频基础方为太冲、合谷、足三里、曲池;腧穴分布以四肢部为主,占53.2%;归经以足厥阴肝经、足少阳胆经、手阳明大肠经、足阳明胃经为主,共计占63.9%;腧穴使用以特定穴为主,占95.7%,高频特定穴有五输穴、原穴、交会穴。结论针灸治疗高血压病基本方为太冲、合谷、足三里、曲池,多从肝经、胃经、大肠经、胆经论治,多选用四肢肘膝关节以下的特定穴。

Objective To study the acupuncture point selection for hypertension according to literature analysis. Methods The clinical literature on acupuncture for hypertension was searched in the China National Knowledge Infrastructure （CNKI）, VIP Database, Wanfang Database and Chinese Biomedical Literature Database （CBM）. The acupuncture prescrilation database was established based on the search results. The points, point combination. Dointdistribution, meridians and the application of specific points were analyzed by relational analysis. Results Totally 285 articles were included, and 285 acupuncture prescriptions and 62 points were involved. The total frequency ofpoint application was 588. The commonly used points were Taichong （ LR3 ）, Quchi （ LI11 ）, Fengchi （ GB20 ）, Hegu （LI4）, Zusanli （ST36） and Baihui （GV20）. The high frequent used acupoints based on acupuncture prescription were Taichong, Hegu, Zusanli and Quchi. The points were mainly distributed in four limbs, accounting for 53.2%. The channel entry was mainly related to Liver Meridian, Gallbladder Meridian, Large Intestine Meridian andStomach Channel, accounting for 63.9%. The commonly used points were specific points, accounting for 95.7%. The frequently used specific points were five shu point, yuan-primary point and crossing point. Conclusion Thebasic acupuncture prescription for hypertension is Taichong, Hegu, Zusanli and Quchi. The channel entry is mainly related to Liver Meridian, Stomach Meridian, Large Intestine Meridian and Gallbladder Meridian. The specific points located below the knees and elbows are commonly used.
