限制吸气流速对吸气肌训练的临床价值 被引量:3

Effects of Limited Airflow Threshold Loading Device for Inspiratory Muscle Training in Patients with COPD

摘要 目的探讨慢性阻塞性肺疾病 (COPD)缓解期患者在阈压力负荷吸气肌训练中限制吸气流速的临床价值。方法作者自行设计一种可限制吸气流速的阈压力负荷吸气肌训练仪 ,应用于临床测试 A、B组共 33例 COPD缓解期患者 ,压力为 2 5 %最大口腔吸气压 (MIP) ,限定吸气流速 <0 .0 5和 <0 .8L/ s,每天训练 40 min。结果 6周后 ,患者 MIP、负荷吸气时间、 12 min最大行走距离均显著增加 (P<0 .0 1) ,常规肺功能无明显改善。 16例对照组应用普通的阈压力负荷吸气肌训练仪 ,结果上述指标均无显著改善 (P>0 .0 5 )。结论在阈压力负荷吸气肌训练中限制吸气流速 ,形成稳定的呼吸方式 ,可在较低的压力负荷下 。 Objective To study The clinical value of limiting flow rate for inspiratory muscle training by threshold loading device. Methods A new device for inspiratory muscle training was designed. Twenty five patients with stable COPD (treatment group) were trained for 20 min every day with the new device loaded with 25 % MIP, airflow being limited to 0 8 L/s. Results Six weeks later, MIP, LT and 12 MD in the treatment group were significantly raised ( P <0 01). There was no change in routine pulmonary function. Oppositely, in the control group (24 cases) with common threshold loading device, all variables showed no significant change ( P >0 05). Conclusion During the process of inspiratory muscle training with threshold pressure, limiting flow rate and forming stable respiratory style could obtain satisfactory clinical effects under a lower pressure load.
