硬膜外自控镇痛对胃癌根治术患者术后机体综合状态的影响

Influence of patient-controlled epidural analgesia for the postoperative comprehensive state of patients undergoing radical operation for gastric cancer

目的:研究硬膜外自控镇痛对胃癌根治术患者术后机体综合状态的影响程度。方法:选取本院收治的78例胃癌根治术患者为研究对象,将78例手术患者随机分为对照组(肌内注射镇痛组)39例和观察组(硬膜外自控镇痛组)39例,然后将两组术前及术后1、3d及7d的免疫、蛋白代谢及炎性应激状态进行比较。结果:观察组术后1、3d及7d的免疫、蛋白代谢及炎性应激状态指标均明显优于对照组,且观察组术后3d的免疫及炎性应激指标均差于其他时间,差异具有统计学意义(P<0.05)。结论:硬膜外自控镇痛对胃癌根治术患者术后机体综合状态的影响更为积极,也更有助于患者术后的尽早康复。

Objective： To study and analyze the influence degree of patient-controlled epidural anal- gesia for the postoperative comprehensive state of patients undergoing radical operation for gastric cancer. Methods.A total of 78 patients underwent radical operation for gastric cancer in our hospital were randomly divided into observation group （patient-controlled epidural analgesia group, n= 39 cases）and control group （analgesia by intramuscular injection group, n= 39 cases）, then the immune, proteometabolism and inflam- matory stress state of two groups before the operation and at first,third and seventh day after the opera- tion were all compared. Results：The immune, proteometabolism and inflammatory stress state indexes of observation group at first, third and seventh day after the operation were all significantly better than those of control group, and the immune and inflammatory stress state indexes of observation group at third day after the operation were all worse than those at other time points （P all 〈0.05）. Conclusion：The influence of patient-controlled epidural analgesia for the postoperative comprehensive state of patients underwent radical operation for gastric cancer is more active, and it is more helpful for the early postoperative rehabili- tation.
