摘要
目的:通过在急诊药局开展品管圈活动,分析处方调剂差错的原因,并采取相关防范措施,降低发药差错率,增强员工参与管理的意识,促进药学服务质量提升,保障患者用药安全。方法:将品管圈活动应用于急诊药局药品的调配过程,回顾性分析2012年造成急诊药局调配差错原因,制订相应对策并实施,评估实施对策的效果。结果:有形成果方面,急诊药局调剂内差由改善前的15.0件/周,下降到改善后的7.25件/周。达标率为115.16%,进步率为51.67%,达到了预期目标;无形成果方面,员工品管手法、解决问题的能力及责任心等方面的评分明显提高。结论:在急诊药局开展品管圈活动是可行的,开展品管圈活动可有效降低调剂差错的发生率,提高了圈员们解决问题的能力,增强了团队合作与互动,提升了整体药学服务水平,保障了患者用药安全。
Objective:To analyze the reasons of prescription errors through the development of products in the emergency pharmacy management circle activities, and to take preventive measures to reduce dispensing error rate; enhance employee participation in management consciousness;and promote pharmacy service quality so as to guarantee the safety of patients. Methods: the product blending process control circle used in emergency pharmacy drugs, retrospective analysis of 2012 resulted in emergency pharmacy dispensing errors reasons, for- mulate corresponding measures, to evaluate the effect of implementing the counter measure. Results: In terms of tangible results,emergency pharmacy dispensing interpolation was dropped from 15 pieces per week to 7. 25 pieces per week. Compliance rate was 115.16% ,and improvement rate was 51.67%, achieving the expected goals. In terms of intangible results, the scores of staff quality management methods, the ability to solve the problem and the sense of responsibility increased obviously. Conclusions: It is feasible to carry out the quality control circle in emergency medicine bureau because it can effectively reduce the incidence of dispensing errors; improve the ability of ring members to solve problems; enhance the team cooperation and interaction;improve the overall level of pharmaceutical service so as to guarantee the safety of patients.
