摘要
目的:研究右美托咪啶对成人胸腔手术单肺通气IL-6和IL-8的影响。方法:以我院2013-01~2014—01收治的选择90例成人胸腔手术患者为研究对象,进行随机分组,研究组为持续输注组,在麻醉诱导后实施o.3pg/(kg·h)右美托咪定静脉泵注,手术结束前30min停止泵注;对照组同期给予等量0.9%氯化钠注射液。对两组患分别在麻醉诱导20min(T0),气管插管后10min(T1),单肺通气30min(T2)、单肺通气90min(T3)及再次双肺通气后10min(T4),等五个时间点血桨中IL-6和IL-8水平进行比较分析。结果:与对照组比较,研究组T1时间点血清IL-6显著低于对照,研究组T1-T。时间点血清IL-8显著低于对照,差异具有统计学意义(P〈0.05)。结论:右美托咪啶可有效抑制成人胸腔手术单肺通气IL-6和IL~8水平的升高,可降低诱发的炎性细胞因子表达。
Objective: To study the effects of right dexmedetomidine on adult thoracic surgery OLV affect IL--6 and IL--8. Methods: We selected 90 adult patients with thoracic surgery from January 2013 to January 2014 treated in our hospital as study objects, and randomly divided into study group and control group. The study group was continuous infusion group, that is, to give intravenous dexmedetomidine infusion after induc- tion of anesthesia in the implementation of 0. 3pg/(kg-h), and to stop pumping 30rain before the end of surgery. The control group over the same period were given the same amount of 0.9 M sodium chloride injec- tion. We analyzed IL--6 and IL--8 levels in blood plasma at five time points, that is, 10rain (T4), 20min (To) for the two groups of patients during anesthesia induction, 10min (T1) after intubation, 30 rain (T2) OLV, and 90 min (T3) OLV. Results: Compared with the control group , the study group time point T4 serum IL--6 was significantly lower than that in control group. The study group time points T1-T4 serum IL -- 8 was significantly lower than that in the control, and the difference was statistically significant (P 〈 0. 05). Conclusion: Right dexmedetomidine can effectively suppress elevated levels of IL--8 single-- lung ventilation in adult thoracic surgery and IL-6, and can reduce the induced expression of inflammatory cytokines.
