儿童金黄色葡萄球菌感染的免疫-血清学研究 被引量:1

Immune-serological study of children with Staphylococcus aureus infection

摘要 目的探讨金黄葡萄球菌(S.aureus)感染患儿血清S.aureus毒素抗体anti-Hla和anti-PVL的变化特点。方法选取本院2010年6月至2012年12月收治的120例患儿为研究对象,跟据S.aureus感染程度情况分为PSS(皮肤软组织初次感染),RSS(皮肤软组织反复感染)及ISS(深度感染)3组,分别测定患者急性期及治疗后恢复期血清S.aureus毒素抗体anti-Hla和antiPVL水平,并以40例健康儿童为对照,进行组间和组内对比分析。结果各组患者急性期和恢复期S.aureus毒素抗体含量均显著高于对照组(P<0.001),且各组之间亦具有显著性差异(P<0.05),RSS组患者anti-Hla、anti-LukS及anti-LukF 3项指标均显著高于其他2组(P<0.05)。PSS患者恢复期血清anti-Hla、anti-LukF水平较急性期均显著降低(P<0.05),LukS水平无显著变化(P>0.05);RSS患者恢复期血清anti-Hla、anti-LukS及anti-LukF水平均较急性期显著降低(P<0.05);ISS患者恢复期血清anti-Hla、anti-LukF 2项指标含量均显著升高(P<0.05),LukS无显著变化(P>0.05)。结论 S.aureus感染与Hla、LukS及LukF密切相关,且anti-Hla、anti-LukS及anti-LukF水平与S.aureus感染程度密切相关,可作为其临床诊断和预后评估的重要指标。 Staphylococcus aureus (S. aureus) is one of most common pathogenic bacteria of human with a high risk of recurrent or invasive infection. This study designed to investigate the variation characteristics of serum anti- Hla and anti-PVL in children infected with S. aureus. Total of 120 cases of children in our hospital were selected and divided into three groups of PSS (primary Staphylococcus aureus), RSS (recurrent Staphylococcus aureus) and ISS (invasive Staphylococcus aureus) according the gradient of infection, while 40 healthy children were recruited as normal eontol. The S. aureus toxin antibody levels of anti-PVL and anti-Hal were detected and analyzed. We found that the anti-toxin levels of anti-Hal, anti-LukS and anti-LukF in patients were all significantly higher than that in healthy children (P〈 0.001), and there was significant difference among patient groups (P〈 0.05): patients in RSS group got higher compared to the other two groups (P〈 0.05). We also found that levels of anti-Hla and anti-LukF were significantly decreased in the convalescent phase of PSS patients compared to the acute phase (P〈 0.05), while there was no difference in anti-LukS (P 〉 0.05); And levels of anti-Hla, anti-LukS, and anti-LukF were all significantly decreased in the convalescent phase of RSS patients compared to the acute phase (P〈 0.05). Further more, levels of anti-Hla and anti-LukF were significantly increased in the convalescent phase of PSS patients compared to the acute phase (P〈 0.05), while there was no difference in anti-LukS (P〉 0.05). Our research result showed that S. aureus infection is closely associated with Hla, LukS and LukF; serum anti-Hla, anti-LukS and anti- LukF level are closely related with infection route and degree.
