摘要
目的探讨一种最新的光学低相干反射测量仪Lenstar LS900对于眼球生物测量的准确性,将其测量值与光学相干生物测量仪IOLMaster的测量结果进行比较。方法对照观察分别由Lenstar LS900和IOLMaster测量的正常人群眼球的眼轴长度(axial length,AL)、前房深度(anterior chamber depth,ACD)、角膜子午线上下的曲率半径(CR1、CR2)和白对白角膜直径(white—to—white distance,WTW)等眼球生物学参数。结果本研究纳入了206个健康的成年个体(206眼),年龄(26.9±15.6)岁。两种测量仪对于AL、ACD、CR1、CR2,和WTW等测量值的相关程度很高。Lenstar LS900与IOLMaster的测量结果比较,AL差值(0.014±0.090)mm(P=0.025),CR.差值(0.006±0.030)mm(P=0.005),CR,差值(0.020±0.040)mm(P〈0.001),均略长;WTW较短差值(-0.110±0.400)mm(P〈0.001),ACD的差异无统计学意义(P=0.554)。除WTW一致性界限为-0.89~0.67mm外,两种测量仪的测量数据具有良好的一致性。结论与IOLMaster相比较,LenstarLS900可提供同样准确、可靠的眼球生物测量数据(除wTw外),并且可提供中央角膜厚度和晶状体厚度数据,因此LenstarLS900在屈光手术和白内障手术领域有很好的应用前景,能满足未来高质量白内障手术的要求。
Objective To evaluate the ocular biometric measurement using a new optical low-coherence refiectometer ( Leustar LS 900 ), and compare the measurements with those obtained from the IOL Master. Methods In this prospective comparative observational study, axial length ( AL), anterior chamber depth ( ACD ), radii of corneal curvature ( CR1 , CR2 ) and white-to-white distance (WTW) were performed using IOLMaster and Leustar LS 900 biometer by the same examiner. Results The study evaluated 205 healthy adults ( 206 eyes), mean age was ( 25.9 ± 5.6 ) years. There was a good correlation for AL, ACD, CRI, CR2 and WTW between two devices. The Lenstar measured a slightly longer AL, mean difference was ( 0. 014 ±0. 090 )mm ( P = 0. 025 ), mean difference of CR, was ( 0. 005 ±0. 030 ) mm ( P = 0.005 ), mean difference of CR2 was(0.020 ±0. 040 ) nun ( P 〈 0. 001 ), and mean difference of WTW was ( - 0. 110 ±0. 400)mm(P 〈0.001 ). The ACD obtained by the two devices was similar (P = 0. 554). Bland-Altman plots displayed good agreement of all parameters except WTW ( limits of agreement was from -0.89 mm to 0.57 mm). Conclusion The Leustar LS 900 provide precise and valid measurements of all parameters analyzed, which are comparable with those of IOLMaster except WTW. The results, coupled with its availability of central corneal thickness and lens thickness measurements make the Leustar LS 900 a promising new instrument for ocular biometry prior to cataract and refractive surgery.
