摘要
目的探讨血清25羟维生素D3[25-(OH)D3]水平与2型糖尿病视网膜病变(diabetic retinopathy,DR)的关系。方法选取144例2型糖尿病患者,分为无DR组(NDR组)50例、非增生期DR组(NPDR组)48例和增生期DR组(PDR组)46例,并选取同期来本院体检的46例健康者为对照组(NDM组)。对每位患者进行临床资料收集及常规生化指标检测,采用电化学发光法检测所有受试者血清25-(OH)D3水平。结果 NDR组患者血清25-(OH)D3浓度为(28.10±10.23)ng·mL-1,明显低于NDM组的(31.59±9.12)ng·mL-1,NPDR组患者血清25-(OH)D3浓度为(25.18±10.56)ng·mL-1,明显低于NDR组,而PDR组患者血清25-(OH)D3浓度为(22.29±10.82)ng·mL-1,又明显低于NPDR组,差异均有统计学意义(均为P<0.05)。Pearson相关分析显示,血清25-(OH)D3水平与病程、HbA1C、血肌酐均呈统计学上的负相关(r=-0.16、-0.19、-0.22,均为P<0.05)。结论 2型DR患者血清25-(OH)D3水平明显降低,且与DR的严重程度存在关联。
Objective To investigate the relationship between serum 25- Hydroxyvitamin D3 [ 25 - (OH) D3 ] concentrations and diabetic retinopathy (DR) in type 2 diabetic patients. Methods A total of 144 patients with type 2 diabetes mellitus and 46 non-diabetic healthy controls were enrolled in this study. All the diabetic patients were divided into three groups :non-diabetic retinopathy group (NDR group ,50 cases), non-proliferative diabetic retinopathy( NDPR group, 48 cases) ,and proliferative diabetic retinopathy( PDR group,46 cases). The 25-(OH) D3 levels in serum were measured by electrochemiluminescence immunoassay. Results NDR group had lower 25- (OH) D3 levels than NDM group [ (28. 10 ±10.23)ng · mL^-1 vs(31.59 ±9.12)ng · mL^-1] (P 〈 0. 05 ). NPDR group had lower 25- (OH) D3 levels than NDR group [ (25.18 ± 10.56 ) ng · mL^-1 vs(28.10 ±10.23)ng · mL^-1] (P 〈0.05) ,and PDR group had lower 25-(OH) D3 levels than NPDR group[ (22.29 ± 10.82)ng · mL^-1 vs(25.18 ±10.56)ng · mL^-1] (P 〈 0.05 ). The result of pearson' s correlation analysis showed that serum 25-(OH) D3 con- centration was negative correlated with course of illness ( r = - 0.15, P 〈 0.05 ), HbA1 C (r= -0.19,P〈0.01)and creatinine(r = -0.22,P〈0.01). Conclusion Serum level of 25- (OH) D3 in type 2 diabetes mellitus patients with DR is obviously lower, and severity of DR is associated with serum 25-(OH) D3 concentration.
