缺血性眼病眼部血流动力学改变临床研究 被引量:7

Clinical study on retrobulbar hemodynamics change in ischemic ophthalmopathy

摘要 目的 通过对缺血性眼病(ischemic ophthalmopathy,IOP)患者患眼进行眼底荧光血管造影(FFA)及彩色多普勒超声诊断仪检查,观察患眼血流动力学改变情况.方法 临床病例对照研究.将咸阳市第一人民医院眼科中心于2010年6月至2013年6月间临床确诊的IOP患者53例(53只眼)纳入IOP组研究,并按照病种不同又分为NAION、RAO、IOS三个亚组.同时,将同病区非IOP患者同侧眼作为对照组进行对照研究.IOP组与对照组分别行FFA及彩色多普勒超声检查,观察臂-视网膜循环时间(A-RCT)、眼动脉(OA)、视网膜中央动脉(CRA)、睫状后短动脉(PCA)的收缩期血流峰值速度(PSV)、舒张末期血流速度(EDV)、阻力指数(RI)等变化,采用统计学方法对检查结果进行分析.结果 IOP组与对照组就其项目进行比较,均差异有统计学意义(P<0.05),包括IOP组A-RCT较对照组延长,OA、CRA、PCA的PSV血流速度降低,RI增高.结论 IOP患者患眼各动脉血流速度降低,阻力增大,进一步导致眼部血流量下降,所以IOP患者患眼发病早期均存在低灌注. Objective To observe the haemodynamics change of eyes with ischemic ophthalmopathy (IOP) by Fluorescence Fundus Angiography (FFA) and Color Doppler imaging.Methods Fifty-three cases (53 eyes) of IOP patients were as experimental group and the homonymy eyes of non-IOP patients from the same endemic area were as control group in our hospital for 3 years.FFA and Color Doppler imaging were performed respectively.Arm-retinal circulation time (A-RCT),ophthalmic artery (OA),central retinal artery (CRA),short posterior ciliary artery (PCA),peak systolic velocity (PSV),end diastolic velocity (EDV) and resistant index (RI) were observed.The results were analyzed by statistical method.Results Comparing with control group,A-RCT was longer,the blood flow rate of OA,CRA and PSV was lower and RI was higher in experimental group.The compare between two groups was statistically different.Conclusions The volume of arterial blood flow in ocular region decrease and the resistance of blood flow increase that lead to the further blood flow rate become lower in IOP patients.Hypoperfusion exist in ocular region of IOP patients in the earlier period.
