全数字化乳腺摄影及MRI对浆细胞性乳腺炎的诊断价值

Value of Full Field Digital Mammography and MRI in the Diagnosis of Plasma Cell Mastitis

目的探讨浆细胞性乳腺炎的全数字化乳腺摄影及MRI检查特征。方法回顾性分析经手术病理证实的37例浆细胞性乳腺炎患者的全数字化乳腺摄影及MRI表现特征。结果 24例表现为局部肿块,4例伴乳头内陷,8例表现为乳头溢液,6例呈急性炎症改变。全数字化乳腺摄影检查37例,有阳性征象者35例,(35/37,94.6%),其主要表现分5型:肿块结节型24例,炎症样型5例,局部结构紊乱型4例,乳晕下导管扩张型2例,阴性表现2例。乳腺MR检查26例,均有阳性发现(26/26,100.0%),其中25例显示强化,多表现为结节样强化2例及条索状强化23例。结论浆细胞性乳腺炎的全数字化乳腺摄影及MRI表现虽无明显特异性,但仍有一定的常见表现,仔细分析其特征,并紧密结合临床资料,可提高诊断准确率,有利于指导外科手术。

Objective To analyze the full filed digital mammographical （FFDM） and MRI findings of plasma cell mas- titis （PCM）and to improve the diagnosis accuracy. Methods The full filed digital mammography and DBMRI findings of 37 patients with plasma cell mastitis confirmed by surgical pathology were analyzed. Results The painful masses were found in 24 patients,while 4 patients with nipple retraction, nipple discharge were found in 8 patients, and acute inflammatory episode were found in 6 patients. FFDM showed positive sign in 35 cases（94.6% ） ,the plasma cell mastitis were classified into five types based on the FFDM findings： mass and nodule type（ n = 24 ）, inflammation like type （n = 5 ）, architectural distortion type （ n = 4）, subareolar ductal ectasis （ n = 2）, negative type （ n = 2 ）. DBMRI showed positive sign in 26 patients（ 100.0% ） , 25 cases showed enhancement, including 2 cases showed nodular enhancement and 23 cases showed linear enhancement. Conclusion Although PCM lack of special FFDM and MRI characteristics, the imaging findings combination with clinical characteristics can help to improve the diagnosis accuracy of PCM .
