胸水细胞DNA/CK双参数法在肺癌诊断中的应用 被引量:2

摘要 目的 探讨DNA/角蛋白 (CK )双参数流式细胞术 (FCM )检测肺癌的价值。方法 对 6 8例癌性胸水和 2 4例结核性胸水患者的同一次胸水进行DNA/CK双参数、DNA单参数及细胞病理学检查。结果 以非整倍体阳性为标准 ,DNA/CK双参数和DNA单参数诊断肺癌的阳性率分别为 94.1%、80 .1% ,其中 5 6例非小细胞肺癌的阳性率分别为 98.2 %、83 .9% ;12例小细胞肺癌的阳性率分别为 75 %、6 6 .7%。而细胞病理学的阳性率则为 6 9.1%。DNA/CK双参数检测法与另两种检测法比较差异有显著性 (P <0 .0 5 ) ;非小细胞肺癌DNA/CK双参数 ,DNA单参数两种检测法比较差异有显著性 (P <0 .0 5 ) ,小细胞肺癌两种检测法比较差异没有显著性 (P >0 .0 5 )。 2 4例结核性胸水经DNA/CK双参数、DNA单参数检测非整倍体均为阴性。 Objective To evaluate DNA/cytokeratin(CK) biparametric method in detection to the lung cancer.Method DNA,DNA/CK of 68 patients with the cancerous pleural effusion and 24 patients with the tuberculotic pleurisy were meassured by flow cylometry(FCM) and cellular pathobiology.Results The presence of aneuploidy was used as a diagnostic criterion,the positive rate was 94.1%, 80.1% ,respectively. The positive rate of non smallcelluar lung cancer was 98.2%,83.9%; The one of smallcelluar lung cancer was 75%,66.7%.the positive rate of cellular pathobiology of effusion was 69.1%.There was a significant difference between the DNA/CK biparametric method and the others(P<0.05).In the non smallcelluar lung cancer,There was a significant difference between the DNA/CK and DNA(P<0.05); In the smallcelluar lung cancer,There was not any significant difference between the DNA/CK and DNA(P> 0.05 ).Conclusion These results suggest that flow cytometric DNA/CK analysis of the pleural effusion may be a valuable adjunctive method in the diagnosis of lung cancer.
