经尿道等离子电切术治疗良性前列腺增生症的临床疗效观察

Clinical efficacy of transurethral plasmakinetic resection of prostate for benign hyperplasia

目的 探讨经尿道等离子电切术（transurethral plasmakinetic resection of prostate,PKRP）治疗良性前列腺增生症（benign prostatic hyperplasia,BPH）的临床疗效.方法 对2012年6月～2013年6月收治的123例良性前列腺增生患者病例进行分析,根据患者所选用的手术方式将患者分为实验组与对照组.实验组63例患者采用经尿道前列腺等离子电切术（PKRP）治疗,对照组60例患者采用经尿道前列腺电切术（transurethral resection of prostate,TURP）治疗,比较两组患者手术时间、术中出血量、切除组织重量以及治疗6个月后最大尿流率的改善.结果 实验组患者手术时间、术中出血量以及切除组织重量分别为（43.27±12.25）min、（96.21±9.45）ml、（24.39±7.45）g明显低于对照组患者的（55.24±11.35）min、（114.33±10.18） ml、（37.15±11.26）g,差异有统计学意义（P＜0.05）.随访6个月发现,实验组患者的最大尿流率[（24.85±6.48）ml/s]明显高于对照组患者[（18.77±5.25）ml/s],差异有统计学意义（P＜0.05）.结论 经尿道前列腺等离子电切术疗效好,是治疗良性前列腺增生的安全术式.

Objective To investigate the clinical efficacy of transurethral plasmakinetic resection of prostate （PKRP） for benign prostatic hyperplasia （BPH）.Methods 146 patients with BPH were prospectively analyzed.These patients were grouped based on their operation method.86 cases as experimental group were treated with PKRP.The other 60 cases as control were treated with transurethral resection of prostate （TURP）.Operation duration （OD）,intraoperative blood loss （IBL）,tissue resection weight （TRW） and maximum urine flow rate （MUFR） improvement 6 months after operation were compared between the two groups.Results OD,IBL and TRW in experimental group were （43.27±12.25） min,（96.21±9.45） ml and （24.39±7.45） g,respectively,which were significantly lower than those of control group （55.24±11.35） min,（114.33±10.18） ml and （37.15±11.26） g,respectively （P〈0.05）.After follow-up for 6 months,MUFR of experimental group （24.85±6.48 ml/s） was found to be significantly higher than that of controls （18.77±5.25 ml/s） （P〈0.05）.Conclusion PKPR isclinieally effective and safe for treatment of BPH.
