锁定钢板与传统钢板治疗跟骨关节内骨折的临床疗效比较 被引量:8

摘要 目的比较锁定钢板与传统钢板治疗跟骨关节内骨折的临床疗效。方法2010年7月1012年7月收治跟骨关节内骨折患者87例(94足),根据钢板选择不同分为锁定钢板组46例(52足)和传统钢板组41例(42足),比较2组患者的手术时间、术后并发症、内固定在位情况、骨折愈合情况、Bohler角恢复程度及踝关节功能(AO—FAS)评分。结果所有患者均获得临床随访,随访时间为12~29个月,中位时间20.6个月。所有患者均获骨性愈合,2组间骨性愈合时间无明显差异(P〉0.05),未发现明显的内固定松动、深部感染、骨髓炎、皮肤坏死等并发症。锁定钢板组在手术时间、Bohler角恢复程度均较传统钢板组改善明显,具有统计学意义[(85.3±6.9)min vs.(91.3±9.4)min、(27.6°±7.1°)vs.(24.8±8.5°),P〈0.05]。术后末次随访锁定钢板组AOFAS评分较传统钢板组改善明显[(87.2±9.5)分vs.(83.1±8.2)分,P〈0.05]。结论锁定钢板较传统钢板在治疗跟骨关节内骨折方面可以有效缩短手术时间、恢复和保持解剖结构,获得更佳的踝关节功能。
