摘要
目的探讨高龄帕金森病患者的生活质量情况,分析影响生活质量的相关因素。方法选取2010-01-2013-01门诊或住院的高龄帕金森患者83例,采用统一的帕金森病评分量表(UPDRSII)、Hoehn-Yahr(H-Y)分期量表、汉密尔顿抑郁量表(HAMD)、帕金森睡眠量表(PDSS)、蒙特利尔认知评估量表(MoCA)等对患者的临床症状进行评估,采用帕金森病生活质量问卷(PDQ-39)对患者的生活质量进行评估,以UPDRSII、HAMD、PDSS为因变量,以影响患者生活质量为自变量进行Logistic回归分析。结果入组患者H-Y分期、UPDRSII评分、HAMD评分、PDSS评分、MoCA评分及PDQ-39评分分别为2.14±0.72、16.04±6.37、13.86±8.14、95.63±13.35、21.36±4.25及44.25±11.73;UPDRSII、HAMD、PDSS是影响患者生活质量的重要因素,UPDRSII、HAMD、PDSS与影响生活质量的OR值分别为2.363、1.782和2.012,P<0.05。结论高龄帕金森患者的生活质量影响严重,抑郁、睡眠障碍及生活能力是影响生活质量的重要因素。
Objective To investigate the quality of life of elderly patients with Parkinson's disease ,and analyze the factors influencing the quality of life. Methods Eighty-three cases of inpatient or outpatient who had Parkinson’s disease from January 2010 to January 2013 were selected ,and examined. The clinical characteristics were assessed by unified Parkinson disease rating scale (UPDRS Ⅱ ) ,Hoehn-Yahr stage ,Hamilton depression scale (HAMD) ,Parkinson’s disease sleep scale(PDSS) , Montreal cognitive assessment (MoCA).The quality of life in patients with Parkinson’s disease was assessed using PDQ-39 questionnaire.Setting UPDRSⅡ ,HAMD ,PDSS as the dependent variables ,setting the quality of life of patients as independent variable. Logistic regression analysis was proceeded. Results HY staging ,UPDRSⅡ score ,HAMD score ,PDSS score ,MoCA score and PDQ-39 scores of patients were 2.14 ± 0.72 ,16.04 ± 6.37 ,13.86 ± 8.14 ,95.63 ± 13.35 ,21.36 ± 4.25 ,44.25 ± 11.73 ,respectively ;UPDRSⅡ ,HAMD ,PDSS were important factors affecting the quality of life of patients ,OR values of UPDRSⅡ ,HAMD ,PDSS affecting the quality of life were 2.363 ,1.782 and 2.012 ,respectively ,P〈 0.05. Conclusion Quality of life of elderly patients with Parkinson’s disease was seriously affected. Depression ,sleep disorders and life skills are important factors affecting the quality of life.
