医院护理民主管理实践——某省级公立医院3年试点改革的个案分析

The Practice of Democratic Management of Nurses:A case study on a tree-year pilot reform in a Provincial Level Public Hospital

目的分析护理民主管理在大型综合医院试点实践的效果,为护理科学管理提供依据和建议。方法以某省级公立医院为例,通过护理民主管理委员会的设置,制定相关职能并实施走动式管理与例会制度相结合的运行机制,试点运行3年。采用问卷调查的方法进行效果评估,评估内容包括护理人员对护理民主管理的满意度以及近4年护士离职情况。结果护理民主管理的满意度最低的是"是否了解民主管理的工作内容",占79.4%,最高的是"对民主管理开展的活动是否有兴趣",占89.7%;护士离职率由4.5%下降至1.9%,差异有统计学意义(χ2=106.978,P<0.000 1)。结论护理民主管理得到了护理人员的普遍认可,调动了其工作积极性,稳定了护理队伍,为探索民主办院新方法提供了初步的实践经验。

Objectives To analyze a pilot study on democratic nursery management in a large general hospital,in order to provide scientific evidence and suggestion for scientific management of nurses.Methods Democratic man-agement of nurses was pilot run in a provincial level public hospital for 3 years.A committee of democratic manage-ment of nurses was set up and authorized with relating functions.A operating mechanism combing management by wandering around and regular meeting was implemented.The evaluation of the effect of this pilot running was con-ducted through questionnaire investigation about nurses＇satisfaction about democratic management and rate of resig-nation in recent 4 years.Results Nurses were the least satisfied with the item of＂awareness of the content of demo-cratic management＂（79.4%）and the most satisfied with ＂interest in activities carried out by democratic manage-ment＂（89.7%）.The resignation rate dropped from 4.5% to 1.9% and the difference was statistically significant （χ^2 =106.978,P 〈0.000 1）.Conclusions Democratic management was publicly recognized by nurses.It aroused nurses＇ initiative,stabilized the team,and provided initial experience from practice for democratic management of the hospital.
