彩色多普勒超声与钼靶摄像诊断乳腺病变的对比研究

A Comparative Study of Diagnosis of Doppler Ultrasound and Mammography in the Breast Lesions

目的:观察对比彩色多普勒超声与钼靶摄像在诊断乳腺病变中的应用价值。方法:选择2011年1月至2014年1月期间我院收治的乳腺疾病患者98例,均先行多普勒超声检查后再进行钼靶摄像检查,并以病理检查结果作为金标准,观察对比两种方法检查的准确性。结果:98例患者通过病理活检后,确诊为乳腺增生32例、乳腺炎28例、乳腺囊肿15例、乳腺癌13例、乳腺纤维瘤10例。彩色多普勒超声检查的准确率为86.73%,钼靶摄像检查的准确率为90.82%,两组检查结果对比差异无统计意义(P>0.05);而乳腺癌及乳腺囊肿检查的准确率对比中,两组差异具有统计学意义(P<0.05)。钼靶摄像对乳腺恶性病变筛查的灵敏度为92.9%,特异度为100.0%,阳性预测值为100.0%,阴性预测值为98.8%,彩色多普勒超声检查对乳腺癌诊断的灵敏度为86.2%,特异度为100.0%,阳性预测值为100.0%,阴性预测值为91.3%。两组对乳腺恶性病变的诊断效度比较有统计学意义(P<0.05)。结论:针对乳腺疾病进行临床检查时,可联合应用彩色多普勒超声与钼靶摄像,可充分发挥优势互补,以此提高诊断准确率及敏感性,降低误诊率,为术前诊断提供更有价值的参考。

Objective：To compare the value of doppler ultrasound and mammography in the diagnosis of breast lesions.Method： Selected 98 cases of breast lesions in our hospital from Jan .2011 to Jan.2014, which were first mammography examed by doppler ultrasonography , and the pathology results as the standard, the accuracy of two methods were compared .Result：Biopsy confirmed the diagnosis of breast hyperplasia in 32 cases, mastitis 28 cases, breast cysts15 cases , breast cancer 13 cases , mammary gland fibroma 14 cases .The accuracy of mammography examination was 90.82%, the accuracy of doppler ultrasonography was 86.73%, There＇s no statistical significance differences in two sets of inspection results （ P〉0.05）;while the comparison accuracy rate of breast cancer and breast cyst examined , There＇s significant difference between two groups （ P〈0.05） .Mammography sensitivity of malignant breast disease screening was 92.9%, the specificity was 100%, the positive predictive value was 100%, the negative predictive value was 98.8%, the sensitivity of Doppler ultrasound in the diagnosis of breast cancer was 86.2%, the specificity was 100%, the positive predictive value was 100%, the negative predictive value was 91.3%.Two groups of malignant breast lesions and validity compared with statistical significance （ P〈0.05 ） .Conclusion： In order to improve the diagnosis efficiency , the combined use of doppler ultrasound and mammography for breast disease , which can decrease the rate of misdiagnosis , guarantee the quality of care for patients .
