输血对重型颅脑损伤患者凝血功能的影响

Coagulation function changes in patients with severe brain injury after blood transfusion

目的探讨输血速度及输血量对重型颅脑损伤患者凝血功能的影响。方法对该院60例重型颅脑损伤患者输血前、后凝血功能指标进行检测并对比分析。结果重型颅脑损伤患者输血后凝血酶原时间(PT)、活化部分凝血酶原时间(APTT)、凝血酶时间(TT)显著增加,并且输血速度较快、输血量较大的患者凝血功能指标的改变更加明显。结论重型颅脑损伤患者输血速度较快、输血量较大会加重患者的凝血障碍,应及时补充血浆,以改善其凝血功能,提高临床输血疗效。

Objective To investigate the coagulation function changes with different speed and different volume of blood transfusion in patients with severe traumatic brain injury .Methods Coagulation parameters of 60 patients with severe traumatic brain injury were detected and analyzed before and after blood transfusion .Results prothrombin time ,activated partial thromboplastin time ,thrombin time was significantly changed in patients with severe traumatic brain injury after transfusion while patients with faster and more blood transfusion ,changed more pronounced .Conclusion Inappropriate speed and volume of blood transfusion in patients with severe traumatic brain injury will deteriorate coagulation dysfunction ,plasma should be promptly added to improve its coagulation function and blood transfusion efficacy .
