骨良性纤维组织细胞瘤的影像学表现

The image features of benign fibrous histiocytoma of bone

目的探讨骨良性纤维组织细胞瘤的影像学特征。方法回顾性分析经手术病理证实的18例骨良性纤维组织细胞瘤的临床及影像学资料。结果 18例患者中股骨患者11例。全部病例的89%(16/18)病灶呈囊状骨质破坏;83%(15/18)病灶呈轻度膨胀性改变,且56%(10/18)具有不完整硬化边;56%(10/18)病灶伴有骨皮质中断。CT表现为等于或稍低于同层面肌肉密度。MRI显示10例患者T1WI呈较均匀等信号,与肌肉信号相仿,T2WI呈高信号或以高信号为主的混杂信号。1例患者行MRI增强检查,呈环形强化。结论骨良性纤维组织细胞瘤影像表现多样,既具有良性骨肿瘤特点又伴有恶性骨肿瘤征象,其影像诊断应该密切结合临床特征。

Objective To explore the image features of benign fibrous histiocytoma of bone（BFH）. Methods The clinical and radiographic data of 18 patients with pathologically confirmed BFH were analyzed retrospeclively. Results 11 lesions localized in femur. Cystic lesions in 16 cases; 83%（15/18） lesions showed mild expansion,and 56% of the cases had discontinue sclerotic rim;Cortical bone interruption could be found in 10 lesions, CT scan ap peared as soft-tissue density similar or lower to that of the skeletal muscle. They showed the signal similar to the muscle on TlWI,and hyperintense signal intensity on T2WI. The wall of the lesion was enhanced in 1 cases on MRI enhancement scan. Conclusion The BFH of bone had benign tumor characteristics, in which, the malignant tumor signs could be seen. The image diagnosis of benign fibrous histiocytoma of bone should closely combined clinical performance.
