暴发性1型糖尿病6例临床资料分析与讨论

Clinical analysis of six cases of fulminant type 1 diabetes mellitus

目的 分析6例暴发性1型糖尿病（FT1DM）的临床资料. 方法 报告2001年1月至2012年12月于复旦大学附属华山医院治疗的6例FT1DM患者资料. 结果 本组病例以起病急骤和严重DKA为特征,平均病程4.83d,入院时平均血糖42.22 mmol/L,平均HbA1c 6.58％,平均FC-P、2 hG-P分别为0.05、0.08 pmol/L.随访9～72个月后复查胰岛β细胞功能无改善. 结论 FT1DM起病急骤、代谢紊乱严重,胰岛β细胞功能受损严重且预后差,应引起重视.

Objective The clinical data with six cases of fulminant type 1 diabetes mellitus （FT1DM）.Methods Clinical and laboratory data of six FT1DM patients in Huashan clinic from 2001 to 2012 were reported and analyzed.Results This disease was characterized by abrupt onset and severe diabetic ketoacidosis.The mean duration from the first hyperglycemic symptoms to first clinic visit was 4.83 d.The mean plasma glucose level was 42.22 mmol/L,and the mean HbA1c level was 6.58％ at first clinic visit.The mean FC-P and 2 hCP were 0.05,0.08 pmol/L at onset.The β-cell function did not recover after nine to seventy two months of follow-up.Conclusion FT1DM patients have clinical feature of abrupt onset,serious metabolic disorders,serious impaired islet β-cell function,poor prognosis,which required special attention by all medical practitioner.
