再次胆道手术术后胆道测压的意义

摘要 目的探讨肝胆管结石术后胆道压力与肝胆管结石病程间的相关性及其可能的作用机制。方法2012年3—6月,对该院94例因肝胆管结石行胆道探查、T管引流患者进行术后压力测定,比较各组测压指标之间的差别。结果再次手术组胆总管压力显著低于初次手术组(P〈0.05)。上消化道造影显示再次手术组中反流发生率明显高于初次手术组(P〈0.05)。结论胆肠反流的发生与胆肠压力差显著性下降有关。Oddi括约肌功能不全可能是肝胆管结石临床进展的重要因素。 [Objective] To investigate the possible action and mechanism of common bile duct pressure(CBDP) in the course of hepatolithiasis. [Methods] Ninety-four patients who had a T tube after operationwere assessed by manometric examination between March 2012 and May 2012. [Results] CBDP were muchlower in repeated operation group than that in control group (P〈0.05). Upper gastrointestinal tract opacificationdetected the incidence of duodenal-biliary reflux patients with hepatolithiasis was higher than that of controlgroup (P〈0.05). [Conclusion] The occurrence of duodenal-biliary reflux is related with the common bile ductpressure. There is a close relationship between sphincter of Oddi hypomotility and clinical progression of hepatolithiasis.
