髌骨内推联合股四头肌锻炼可有效改善老年髌股关节炎的症状和功能

Patellar inward pushing combined with quadriceps exercise improves patellofemoral osteoarthritis symptom and function

目的观察髌骨内推和股四头肌功能锻炼对老年髌股关节炎患者症状和功能的影响。方法髌股关节炎患者在膝关节伸直状态下,对髌骨进行内推,髌骨活动后,进行股四头肌功能锻炼。对患者治疗前和后的IKDC评分、膝关节非负重状态下的屈曲度进行比较。所有的患者内推前和6个月后进行了膝关节的正侧位片检查。结果 2010年10月至2011年12月,符合髌股关节炎诊断的患者168例共203个关节,患者都进行了髌骨内推和股四头肌功能锻炼。所有的患者都得到了最短6个月的随访,平均随访(10±2.5)个月。治疗前、后IKDC评分比较有显著性差异,治疗1月后IKDC评分高于6月后。膝关节非负重状态下屈曲度检查显示治疗前、后比较有显著性差异(P<0.05)。X-ray膝关节侧位片显示Insall-Salvati指数治疗后没有显著改变(P>0.05)。结论髌骨内推和股四头肌功能锻炼能够显著改善髌股关节炎的症状和功能。

Objective To observe the role of pushing the patella medially and the quadriceps functional exercising in improving the symptoms and functions of the elderly patellofemoral arthritis . Methods Pushing the patellas of the patients suffured from patellofemoral arthritis in relax and straight state once a week for four weeks .After the patella have been mobilized , quadriceps function exercise is performed every day .The international knee documentation committee knee action form （ IKDC） score and the non-weight-bearing knee flexion before and after treatment is compared .All the patients have been＆amp;nbsp;preformed the knee anterior -posterior and lateral x -ray plain film checking before and six months after treatment.Result Form October 2010 to December 2011, 203 joints of 168 patients diagnosed as patellofemoral arthritis were included into our study .The patients were treated by pushing patella medially and quadriceps function exercising .All the patients were followed -up for at least six months （ average 10 ±2.5 months）.The IKDC scores have significant difference before and after treatment .The IKDC scores of one month are more higher than six months after treatment .The non-weight-bearing knee flexion have significant difference before and after treatment .The knee lateral x-ray plain film examination showed the Insall-Salvati index have not changed after treatment .Conclusion The patella medial pushing and quadriceps function exercising greatly improved the symptoms and functions of elderly patellofemoral arthritis.
