临床护理路径在妇产科择期手术患者中的应用效果评价

Evaluation of the application effect of clinical nursing pathway for patients of gynecology and obstetrics receiving selective operation

目的研究临床护理路径（cNP）在妇产科择期手术患者护理过程中的应用效果。方法以本院2013年1月-2014年1月妇产科收治的110例择期手术患者为研究对象，随机分为A组与B组。所有患者均积极进行基础治疗与护理，A组患者在此基础上增加临床护理路径内容，比较两组患者的术前心理状态、术中应激水平、术后生活质量评分和护理满意度。结果A组的术后焦虑、抑郁、恐惧和人际关系敏感得分分别为（2．03±0．48）、（1．84±0．27）、（1．74±0．30）、（1．92±0．31）分，均显著低于B组的（2．58±0．54）、（2．35±0．38）、（2．43±0．49）、（2．17±0．44）分（P〈0．05）；A组的术中应激水平亦低于B组（P〈0．05）；A组的术后睡眠质量、疼痛水平、躯体症状和心理健康水平得分分别为（84．58±5．47）、（45．24±6．69）、（65．69±5．02）、（89．37±3．21）分，B组分别为（79．42±3．02）、（54．48±8．65）、（78．74±6．63）、（84．36±5．46）分，A组的术后生活质量显著高于B组（P〈0．05）；A组的护理满意度（85．45％）高于B组（67．27％）（P〈0．05）。结论CNP可以在妇产科择期手术患者的护理中发挥积极作用，其不仅可以降低患者的心理和生理应激水平。还可以改善患者的生活质量和提高护理满意度。

Objective To explore the application effect of clinical nursing pathway （CNP） for patients of gynecology and obstetrics receiving selective operation. Methods 110 patients who were admitted to the department of gynecology and obstetrics in our hospital from January 2013 to January 2014 and received selective operation were selected as research subjects.They were randomly divided into the group A and the group B,and all patients were given active basic treatment and nursing.Patients in the group A also received CNP on the basis of the basic nursing. The preoperative mental conditions,stress level during operation,postoperative life quality and satisfaction degree of nursing between the two groups were compared. Results Score of postoperative anxiety,depression,fear and sensitivity to interpersonal relations in the group A were （2.03±0.48）,（1.84±0.27）,（1.74±0.30） and （1.92±0.31） points respectively,which were all significantly lower than those of （2.58±0.54）,（2.35±0.38）,（2.43±0.49） and （2.17±0.44） points in the group B respectively （P〈0.05）.The stress level in the group A during operation was also lower than that in the group B （P〈0.05）.Score of sleeping quality, pain level, somatic symptoms and level of mental health in the group A were （84.58±5.47）, （45.24±6.69）, （65.69±5.02） and （89.37±3.21） points respectively,and those in the group B were （79.42±3.02）,（54.48±8.65）,（78.74±6.63） and （84.36± 5.46） points respectively,and the postoperative life quality in the group A was significantly better than that in the group B （P〈0.05）.The satisfaction rate in the group A （85.45%） was higher than that of 67.27% in the group B （P〈0.05）. Conclusion CNP has an active effect in the nursing for patients in department of gynecology and obstetrics receiving selective operation,which is not only able to reduce patients＇ mental and physiological stress level,but also is able to improve patients＇life quality and satisfaction degree of nursing.
