p27和cyclin D1在甲状腺乳头状癌中的表达及其与淋巴结转移的关系 被引量:2

摘要 目的:研究p27、细胞周期蛋白D1(cyclin D1)在甲状腺乳头状癌中的表达及其与甲状腺乳头状癌淋巴结转移的关系。方法:采用免疫组织化学EnVision法检测60例甲状腺良性病变(其中结节性甲状腺肿30例,甲状腺腺瘤30例)和92例甲状腺乳头状癌(其中无淋巴结转移者50例,淋巴结转移者42例)组织中p27、cyclin D1的表达情况。结果:p27在甲状腺良性病变和甲状腺乳头状癌中的阳性表达率分别为91.7%、62.0%,而cyclin D1的阳性表达率分别为28.3%、63.0%,差异均有统计学意义(P<0.01)。p27在无淋巴结转移和有淋巴结转移的甲状腺乳头状癌中的阳性表达率分别为76.0%、45.2%,而cyclin D1的阳性表达率分别为50.0%、78.6%,差异均有统计学意义(P<0.01)。有淋巴结转移的甲状腺乳头状癌中p27和cyclin D1的表达呈负相关(r=-0.46,P<0.01)。结论:甲状腺乳头状癌中p27低表达、cyclin D1高表达,且二者与甲状腺乳头状癌的淋巴结转移相关,并对预后判断有指导意义。 Objective: To investigate the expression of p27 and cyclin D1 in papillary thyroid carcinoma and their relationships with lymph node metastasis of papillary thyroid carcinoma. Methods: By immunochemical EnVision method, the expression levels of p27 and cyclin D1 were detected in 60 cases with thyroid benign lesions(30 cases with nodular goiters and 30 cases with thyroid adenomas) and 92 cases with papillary thyroid carcinoma(50 cases with no lymph node metastasis and 42 cases with lymph node metastasis). Results: The positive ratios of p27 were 9l. 7% and 62.0% in benign and malignant lesions of thyroid. The positive ratios of cyclin D1 were 28.3% and 63.0% in benign and malignant lesions of thyroid. There were statistical differences between the two groups(P〈0.01). The positive ratio of p27 was 76.0% in no lymph node metastasis group,which was 45.2% in lymph node metastasis group. The positive ratio of cyclin D1 was 50.0% in no lymph node metastasis group, which was 78.6% in lymph node metastasis group. There were also statistical differences between the two groups(P〈0.01). There was negative correlation between the expression of p27 and cyclin D1 in lymph node metastasis group(r = - 0.46, P〈0.01). Conclusions: Low expression of p27 and over-expression of cyclin D1 are found in papillary thyroid carcinoma, compared with thyroid benign lesions. P27 and cyclin D1 are also associated with lymph node metastasis of papillary thyroid carcinoma.
