社区相关性多重耐药菌院内传播防控措施应用研究

Clinical prevention and control measures research of community-associated multi-drug resistant organisms infection in hospital

目的探讨有效的社区相关性多重耐药菌在院内传播防控措施。方法根据临床常规送检微生物培养结果,对发现的社区相关性多重耐药菌感染患者采取如下院内传播防控措施,①加强监测:由专人负责,每天针对临床常规微生物培养结果进行目标性监控;②临床干预:以"多重耐药临床通知单"内容进行临床干预,同时展开以相关患者为中心的培训;③全程监控:医院感染管理科专职人员监测到目标菌后,每天观察患者病情,3 d后现场督查防控措施落实情况;该防控措施持续到连续2个送检标本阴性或出院。结果在本防控措施实践17个月期间社区相关性多重耐药菌院内传播病例为零。结论本研究的社区相关性多重耐药菌院内传播防控措施有效。

Objective To explore the effective measures for the prevention and control of community-associated multi-drug resistant organisms （CA-MDROs） infection in hospital. Methods According to the results of routine clinical microbiolog- ical culture,the patients with CA-MDROs infection were selected, and the following actions were taken to prevent and con- trol the spread of CA-MDROs infection ： （!）To strengthen the monitoring ： By the specialist is responsible, daily routine mi- crobiological culture results were targeted for clinical monitoring; （~）Clinieal intervention： ＂CA-MDROs notice＂ clinical intervention,launched by relevant patients as the center of training at the same time;（~）Whole-proeess monitoring： mem- bers in Department of Nosocomial Infection observed the patients everyday if targeted CA-MDROs were detected and be- gan monitoring implementation of the measures for the prevention and control of CA-MDROs infection; these performance was continued until double negative results were detected or to the discharge. Results There was no hospital-acquired CA-MDROs infection cases in the practice of these prevention and control measures for 17 months. Conclusion The measures is efficacious in the prevention and control of CA-MDROs infection spread in the hospitals.
