我院静脉用药集中调配中心常见不合理医嘱分析

The analysis of common irrational medical orders by pharmacy intravenous admixture services in our hospital

目的：分析我院住院患者静脉输液不合理医嘱情况，为临床合理用药提供参考。方法对我院静脉用药集中调配中心2012~2013年2613267条静脉医嘱中的4263条不合理医嘱进行回顾性统计分析。结果不合理医嘱类型主要涉及：电脑录入错误、溶媒选择不当、溶媒用量不当、配伍禁忌、用药频次不当、用药剂量不当等情况。结论通过药师对临床医嘱的干预，不合理医嘱发生率逐渐降低，促进了临床合理用药，提高了药学服务的质量。

Objective To analyse the irrational medical orders of the inpatients with venous transfusion in our hospi-tal,and provide reference for clinical rational use of drugs. Methods 4 263 irrational medical orders out of total 2 613 267 vein orders from the year 2012 to 2013 were retrospectively analyzed to intravenous drug use in our hospital. Results The unreasonable medical types mainly involved：computer entry error,improper selectiong of solution,the improper amount of solvent,incompatibility,improper frequency of use,improper dosage etc. Conclusion With the intervention of the pharma-cists in clinical medical orders,the incidence of the unreasonable medical orders decreased,which not only promoted the clinical rational use of drug but also improved the quality of pharmaceutical care.
